摘 要:目的:探讨心型脂肪酸结合蛋白(H-FABP)和缺血修饰白蛋白(IMA)在急性冠脉综合征(ACS)诊断中的应用价值。方法:检测135例胸痛患者H-FABP、IMA、心肌肌钙蛋白(cTnT)水平,按发病时间分为<3h、3~6h、6~12h时段,计算各项指标诊断ACS的灵敏度及特异度。结果:H-FABP、IMA在<3h时间段诊断ACS的灵敏度分别为40.0%和52.0%,在3~6h时间段的灵敏度分别为53.3%和50.0%,H-FABP联合IMA灵敏度分别为64.0%和75.0%,均优于cTnT(P<0.001),在6~12h段H-FABP与cTnT差异无显著性,IMA灵敏度明显降低。三项指标诊断ACS的特异度比较差异无统计学意义。结论:H-FABP、IMA对ACS诊断有较高的灵敏度和特异度,H-FABP联合IMA可明显提高ACS早期诊断的敏感性。Objective To explore diagnostic value of heart-type fatty acid binding protein(H-FABP) and ischemia modified albumin(IMA) for acute coronary syndrome(ACS). Methods To observe 135 patients with acute chest pain. Data of H-FABP,IMA and cardiac troponin (cTnT) were detected within 12 hours after the symptoms appeared,their diagnostic sensitivities for ACS were evaluated. Results The sensitivities of H-FABP and IMA within 3 hours were 40.0% and 52.0% respectively,in 3 ~ 6 hours were 53.3% and 50.0% respectively. The sensitivity of combination of H-FABP and IMA was 64.0%(within 3 h) and 75.0%(in 3 ~ 6 h),which was higher than that of cTnT(P 〈0.001). In 6 ~ 12 hours,there was no significant difference among H-FABP,IMA and cTnT in specificity of ACS. Conclusion H-FABP and IMA has more sensitivity and specificity for early diagnosis of ACS,they can improve the early diagnostic sensitivity of ACS significantly.
