摘  要:目的:观察抗肝衰复方(KGSFF)对急性肝衰竭大鼠防护作用及肿瘤坏死因子(TNF-α)表达的影响,探讨KGSFF对大鼠肝衰竭的作用机制。方法:健康雄性Wistar大鼠110只,随机分为正常对照组10只,模型组20只、促肝细胞生长素(PHGF)阳性对照组20只、茵陈蒿汤(YCHT)组20只、KGSFF小剂量组20只、KGSFF大剂量组20只。除正常对照组外其余各组大鼠分别给予D-氨基半乳糖(D-GalN)800mg/kg和内毒素脂多糖(LPS)0.1mg/kg的剂量,腹腔注射1次成模,正常对照组大鼠给予等量生理盐水腹腔注射。除模型组和正常对照组外各组大鼠均于造模后2小时用药,按每次1.5ml/100g,每天灌胃两次。模型组和正常对照组大鼠均分别给予生理盐水灌服。造模后72小时取材。观察造模后72小时内大鼠病死率。检测血清丙氨酸转氨酶(ALT)、天门冬氨酸转氨酶(AST)、总胆红素(TBil)及肿瘤坏死因子-α(TNF-α)水平。HE染色观察肝组织病理变化,免疫组织化学染色法检测肝组织TNF-α水平。结果:KGSFF大剂量组大鼠病死率及血清ALT、AST、TBil含量降低,肝细胞损伤程度显著改善,血清及肝组织TNF-α水平明显降低,与模型组相比P<0.01或P<0.05。结论:KGSFF对急性肝衰竭大鼠具有明显的防护作用,其机制可能与抑制TNF-α产生,降低内毒素血症,改善肝功能有关。Objective:To observe the effects of Kangganshuai Fufang(KGSFF)on the protective function and the expression of TNF-α and to explore its mechanism related to relieve the damage and promoting regeneration in rats with laboratory acute hepatic failure.Methods:One hundred and ten Wistar male rats were randomly divided into six groups: normal control group(n=10),model control group(n=20),PHGF control group(n=20),YCHT group(n=20),KGSFF group of small dose(n=20),KGSFF group of large dose(n=20).All rats were injected with D-GalN 800mg/kg and LPS 0.1mg/kg except the normal group injected with the salt solution intraperitoneally.In 2 hours after model being made,the rats of nomal control group and model control group were treated with salt solution,KGSFF low and high dosages,YCHT,PHGF were treated respectively at a dosage of 1.5ml/100g twice every day for 3 days.And 72 hours later,the blood and hepatic tissues were collected.The mortality of the rats were observed in 3 days.The liver function including serum level of ALT,AST,TBil and TNF-α were measured by biochemical methods.The pathological morphologies of hepatic tissues were examined with HE staining.The level of TNF-α in hepatic tissues were measured by immunohistochemical staining.Results:Compared with those in the model rats,the mortality of the rats and serum ALT,AST,TBil and TNF-α level remarkablely decreased,the cell necrosis infiltration were significantly improved,and the level of TNF-α in hepatic tissues also droped in the KGSFF group of large dose(P0.01 or P0.05).Conclusion:The KGSFF has an outstanding protective function on the rats with acute hepatic failure,and its mechanism may be related to decreasing TNF-α,relieving the intestinal endotoxemia and improving the liver function.
