低分子肝素联用银杏达莫注射液治疗慢性肺心病急性加重期疗效观察

摘　　要：目的探讨低分子肝素联用银杏达莫注射液治疗慢性肺心病急性加重期的疗效及药物安全性。方法选取85例慢性肺源性心脏病急性加重期患者,随机分成治疗组43例和对照组42例。治疗组在常规综合治疗的基础上给予低分子肝素钠0.4ml皮下注射,1次/12h,并联用银杏达莫注射液20ml加入5%葡萄糖溶液250ml静脉滴注,连用7d;对照组给予与治疗组相同的常规综合治疗。观察两组患者临床表现(呼吸频率、紫绀、心率、双下肢水肿、肝颈静脉回流征、咳嗽、咳痰、哮喘症状)的好转时间、心功能、氧分压、二氧化碳分压、血纤维蛋白原、血细胞比容。结果治疗组患者心功能、血纤维蛋白原、治疗后临床症状与体征的好转时间、血气分析指标好转时间、临床疗效与对照组比较,差异均有统计学意义(P<0.05),且无严重不良反应。结论低分子肝素联用银杏达莫注射液治疗慢性肺源性心脏病急性加重期疗效显著,具有良好的安全性。Objective To investigate the effectiveness and safety of low molecular heparin combined with ginkgo leaf extract and dipyridamole injection on acute exacerbations of chronic corpulmonale.Methods Totally 85 patients who were at the acute exacerbation stage of chronic corpulmonale were randomly divided into combination group [treated with low molecular heparin（0.4ml Ih q12h）combined with ginkgo leaf extract and dipyridamole injection（20ml in 250 ml 5%glucose injection,qd）,n=43]and conventional group（treated with conventional therapy,n=42）.The clinical manifestations including respiratory rate,cyanosis,heart rate,edema of the lower extremities,hepatojugular reflux,cough,and wheezing were observed.The remission time,cardiac function,PaO2,PaCO2,blood fibrinogen,and hematocrit were also recorded.Results Cardiac function,blood fibrinogen level,remission time,time of the blood gas became improve,and clincal effectiveness were significantly superior in the combination group than in conventional group（allP〈0.05）.No severe side effects were observed in both groups.Conclusion Low molecular heparin combined with ginkgo leaf extract and dipyridamole injection is safe and effective in treating acute exacerbations of chronic corpulmonale
