呼吸道合胞病毒感染时Th1和Th2细胞因子失调与内源性血浆皮质醇的关系

摘　　要：目的通过测定急性呼吸道合胞病毒(RSV)感染时内源性皮质醇浓度和Th1/Th2类细胞因子及TGF-β1,揭示RSV感染时内源性血浆皮质醇的变化及其与Th1/Th2细胞因子之间的关系。方法选择42例小于1岁的急性RSV感染的婴儿,其中重症21例,轻症21例。同时选择21例同期健康婴儿做为对照组。ELISA方法检测培养上清中细胞因子IL-12、IFN-γ、IL-4、IL-5、IL-10和TGF-β1水平,放射免疫分析法检测血浆皮质醇的浓度。结果重症组IL-12和IFN-γ水平显著低于轻症组(P<0.05);轻症组IL-12和IFN-γ水平显著低于对照组(P<0.05);重症组TGF-β1和血浆皮质醇水平显著高于轻症组(P<0.05);轻症组TGF-β1和血浆皮质醇水平显著高于对照组(P<0.05);IL-4、IL-5和IL-10在3组中没有差别(P>0.05)。IL-12和IFN-γ水平与血浆皮质醇水平呈负相关。结论 RSV感染可导致患儿IL-12和IFN-γ水平降低,从而出现炎症反应,可能与血浆皮质醇和TGF-β1水平增高有关。【Objectives】 To observe the expression of plasma cortisol,Th1/Th2 cytokine and TGF -β1,investigate the relationship between them.【Methods】 42 definitely diagnosed cases of infant with RSV infesction were selected for study were divided into two groups,severe group （21） and mild group （21）,as well as 21 cases healthy infant for control group.The levels of IL-12,IFN-γ,IL-4,IL-5,IL-10 and TGF-β1 were determined by ELISA,the levels of plasma cortiso were determined by RIA.【Results】 The levels of IL-12 and IFN-γ in serve group were significantly lower than that of the mild group （P 0.05）.The levels of IL-12 and IFN-γ in mild group were significantly lower than that of the control group （P 0.05）.The levels of TGF-β1 and plasma cortisol in serve group were significantly higher than that of the mild group （P 0.05）.The levels of TGF-β1 and plasma cortisol in mild group were significantly higher than that of the control group （P 0.05）.There were no significant differences between three groups which the levels IL-4,IL-5 and IL-10 （P 0.05）.There is a negative correlation between the levels of IL-12 and IFN-γ and plasma cortisol.【Conclusion】 RSV infection may induce the low level of IL-12 and IFN-γ,accordingly show up a inflammatory reaction,where may have a relationship with the high level of plasma cortisol and TGF-β1.
