改良Hardinge小切口全髋关节置换术早期疗效对比研究

摘　　要：目的报告改良Hardinge小切口与传统Hardinge切口全髋关节置换术早期疗效的比较结果。方法 2008年9月～2009年9月运用改良Hardinge小切口施行全髋关节置换术26例,并与同期运用传统Hardinge切口26例的术中和术后指标进行比较。结果改良Hardinge小切口组患者在切口长度、失血量(包括术中出血量和术后24h引流量)和术后疼痛等方面明显优于传统Hardinge切口组(P<0.05)。手术时间及Harris评分等方面两组差异无显著性。结论改良Hardinge小切口具有创伤小、出血少和恢复快等诸多优点,但同时应当注意该手术方法的适应证。【Objective】 To report therapeutic result of total hip arthroplasty （THA） through minimal incision via modified Hardinge approach （MHA） and its early comparison with traditional Hardinge incision operation.【Methods】From September 2008 to September 2009,26 cases of THA were performed through minimal incision via MHA.26 patients were performed THA through traditional Hardinge incision in the corresponding period.Data of operation and postoperation has been analysed and compared.【Results】 The result of incision length,bloodloss and postoperative pain achieved through MHA were significantly better than that of traditional Hardinge incision （P 0.05）.There was no significant difference of operation time and Harris score between two groups.【Conclusion】 The minimal incision via MHA is an ideal method for total hip arthroplasty with less surgical trauma,less blood loss and rapidly recovery while the surgical indications should be noted.
