电视胸腔镜辅助小切口配合超声刀治疗自发性气胸40例

摘　　要：目的探讨电视胸腔镜辅助小切口(VAMT)配合超声刀治疗自发性气胸的可行性和优越性,总结临床经验。方法对2007年5月至2010年3月我科收治的40例自发性气胸行电视胸腔镜辅助小切口配合超声刀肺大疱切除术及胸膜固定术的临床资料作回顾性分析。结果手术时间90～180 min,术后胸管留置时间3～12 d,住院时间8～20 d;术后肺漏气2例,复张性肺水肿1例。患者均痊愈出院,随诊时间6～30个月,平均18个月,无复发病例。结论电视胸腔镜辅助小切口配合超声刀治疗自发性气胸具有手术时间短、出血少、术后恢复快及住院时间短等优势。Objective To explore the possibility and advantages of the treatment of video-assisted minithoracotomy（VAMT） and ultracision-harmonic scalpel in spontaneous pneumothorax and to summarize its clinical experience.Methods Clinical data of 40 patients with spontaneous pneumothorax performed VAMT and ultracision-harmonic scalpel treatment of excision of bullae lung and pleurodesis were analyzed retrospectively in the Ningde Municipal Hospital from May 2007 to March 2010.Results The operation took 90-180 min,with the chest tube indwelled for 3-12 days and stayed in the hospital for 8-20 days.Air leakage occurred in two cases and reexpansion pulmonary edema occurred in one case.All patients recovered well.When followed for 6-30 months,there were no relapse during an average length of 18 months.Conclusion The VAMT and ultracision-harmonic scalpel treatment in spontaneous pneumothorax has advantages of shorter operation time,less bleeding,rapid recovery,short hospitalizing time.
