自裁聚丙烯非标准网片治疗重度女性盆底功能障碍性疾病的中期疗效观察

摘　　要：目的探讨在女性盆底功能障碍性疾病中采用自裁Gynemesh聚丙烯非标准定形网片行手术重建盆底功能的小样本中期疗效。方法对8例患有不同腔室Ⅲ期以上盆底功能障碍性疾病的患者采用自裁Gynemesh聚丙烯非标准定形网片试行阴道前壁和(或)后壁修补术,有尿失禁者同时行尿道中段悬吊带术,术后进行1～4年的中期随访。结果 8例患者均顺利完成手术,无血肿发生,无尿道、膀胱及直肠损伤。手术平均时间95.7min(55～160min),术中平均出血125.7ml(20～700ml)。术后恢复良好,平均术后住院时间5.9d(5～7d)。平均随访32.3个月(12～45个月),1例出现术后尿潴留,1例发生术后轻微感染征,无侵蚀、复发。结论自裁Gynemesh聚丙烯非标准定形网片应用于盆底重建,方法简单,手术时间较短,安全有效,经济、节约资源,可供临床选用。Objective To investigate the medium term effect of pelvic floor reconstruction with Gynemesh （Polypropylene） on small samples with the pelvic floor dysfunction （PFD）. Methods Eight women suffered from PFD in or beyond grade Ⅲ in different pelvic cavity underwent transvaglnal anterior and/or posterior pelvic floor repair with Gynemesh if patients with urinary incontinence were taken mid - urethral sling. About 1-4 years periodic post - operational follow - up was conducted. Results The operations were performed successfully in all cases and no complications such as injuries of bladder, urethra or rectum, no hematoma occurred in 8 cases. The average dura- tion of operations was 95.7 min（55 - 160 min）. The mean operational bleeding volume was 125.7ml （20 -700 ml）.All eases recovered well after the operation. The average length of post - operational hospitalization was 5.9 d （5 - 7 d）. The mean duration of post - operational follow -ups was 32. 3 months （12 - 45 months）. One case of surgical site infection and 1 case of uroschesis occurred. No recurrence or erosion occurred according to the follow - up. Conclusion The application of Gynemesh in the females＇ pelvic floor reconstruction was simple, safe and effective so it could shorten the time needed for surgical operations. This technique shows high potential in correcting pelvic organ prolapse.
