摘 要:目的:建立近似人类新生儿窒息的病理过程的新生鼠窒息心肌损伤模型,观察心肌细胞凋亡及使用三七总皂甙(TPNS)后心肌细胞凋亡的变化。方法:出生7天SD大鼠88只,雌雄不拘,随机分为对照组(n=8只),窒息组(n=40只),窒息+TPNS(n=40只)。将大鼠分别放入55ml的磨口瓶中,瓶内放入0.005kg的钠石灰,待大鼠安静后,窒息组,窒息+TPNS组塞紧瓶塞0.5h,取出吸氧120min后,放回母鼠身边继续喂养,分别于窒息后6h、24h、48h、72h、7天后处死;对照组放入55ml的磨口瓶中0.5h不塞瓶塞,取出吸氧120min后放回母鼠身边继续喂养,6h后全部处死。采用TUNEL法测定心肌细胞凋亡,每组各随机抽取一个标本做电镜片HE染色的光镜病理片。结果:①窒息组、窒息+TPNS组各时段的心肌病理切片有明显的心肌细胞坏死,炎性细胞浸润和出血;电镜切片下有心肌细胞线粒体普遍肿胀,细胞核肿胀,染色质边集,肌纤维融合,Z线不清,线粒体空泡,脊模糊稀疏;对照组光镜和电镜病理切片均正常;②窒息组,窒息+TPNS组各时段心肌细胞凋亡率明显高于对照组(P<0.01),有明显统计学差异;③窒息组各时段心肌细胞凋亡率明显高于窒息+TPNS组(P<0.05)。结论:窒息时心肌细胞凋亡率上升是心肌损伤的一种形式;使用三七总皂甙后能明显减轻心肌细胞凋亡率,提示三七总皂甙对窒息所致心肌损伤有明显保护作用。Objective:To establish an animal model similar to the pathological course of myocardial injury induced by human neonatal asphyxia,observe apoptosis of myocardial cells and effect of total panax notoginseng saponins(TPNS)on apoptosis of myocardial cells.Methods:88 SD rats aged 7 days were selected and divided into control group(n=8),asphyxia group(n=40)and asphyxia+TPNS group(n=40)randomly;the rats were put into 55 ml ground-glass stoppered flask,then 0.005 kg soda lime was put into the ground-glass stoppered flask,bottle stoppers were stuffed up in asphyxia group and asphyxia+TPNS group after the rats were silent,then the rats were removed to inhale oxygen for 120 minutes,they were fed with their mothers,the rats were sacrificed at 6 hours,24 hours,48 hours,72 hours and 7 days after asphyxia;the rats in control group were put into 55 ml ground-glass stoppered flask for 0.5 h,bottle stoppers were not stuffed up,then the rats were removed to inhale oxygen for 120 minutes,they were fed with their mothers,the rats were sacrificed at 6 hours after asphyxia;TUNEL method was used to detect apoptosis of myocardial cells,one specimen was selected from each group to make pathological sections of HE staining under light microscope.Results:In asphyxia group and asphyxia+TPNS group,obvious necrosis of myocardial cells,inflammatory cell infiltration and hemorrhage were found in pathological sections;general swelling of mitochondria,swelling of cell nuclear,chromatin margination,fusion of muscle fiber,faint Z line,mitochondrial cavitation,faint and rare mitochondrial cristae were seen under electron microscope;the pathological sections under light microscope and electron microscope in control group were normal;the apoptosis rates of myocardial cells at various time periods in asphyxia group and asphyxia+TPNS group were significantly higher than those in control group(P0.01);the apoptosis rates of myocardial cells at various time periods in asphyxia group were significantly higher than those in
