子宫肌层金属异物取出新方法的临床应用

摘  要:目的探讨三维立体坐标器和特制异物钳在子宫肌层内细小金属异物(断裂节育环、断裂缝针)取出术中的应用价值,来寻找取出子宫肌层内细小金属异物安全有效的新方法。方法先采用三维立体坐标器行子宫肌层内金属异物精确定位后,再用带磁性的配套特制异物钳(均获国家发明专利)钳取88例子宫肌层内金属异物。结果 88例子宫肌层内金属异物,其中25例为断裂残留于子宫肌层内金属IUD,63例嵌顿于子宫肌层内的断裂缝针,全部一次取出,平均手术时间为10.3 min,无子宫大量出血、子宫穿孔等手术并发症发生。结论三维立体坐标器和特制异物钳应用于子宫肌层内细小金属异物取出术,是又一种定位精确,取出成功率高,手术时间短,并发症少,无严重并发症的微创手术。Objective To study the application of three-dimensional locator and magnetic foreign body forceps in removing tiny metallic foreign body( such as broken intrauterine device,broken needle,et al) in the uterine muscle,and seek a safe and effective way of removing tiny metallic foreign body in the uterine muscle. Methods Located precisely by using three-dimensional locator,88 cases of metallic foreign bodies in the uterine muscle( 25 cases of the broken intrauterine device and 63 cases of broken needles) were removed with magnetic foreign body forceps( Both tools are nationality inventive patent). Results All metallic foreign bodies in the uterine muscle of 88 cases were successfully removed by using three-dimensional locator and magnetic foreign body forceps, and the mean operation time was 10.3 minutes. There was no complication such as uterine hemorrhage and perfora- tion. Conclusion To remove the tiny metallic foreign body in uterine muscle by using three-dimensional locator and magnetic foreign body forceps is an effective operation with a precise locating, high successful rate, short operation time, minimal invasion and few serious invasions.
