摘　　要：目的:探讨前路双钉棒内固定系统在治疗胸腰段脊柱结核的疗效。方法:2007年1月-2009年10月,对37例胸腰段脊柱结核病人行经侧前路病灶清除、减压、植骨、双钉棒系统内固定术。其中3个椎体破坏塌陷6例,2个椎体12例,1个椎体19例,均有不同程度的后凸畸形。术前抗结核治疗2-4周。侧前路彻底清除病灶,撑开矫正后凸畸形,减压后行植骨,侧前路双钉棒系统内固定。术后继续应用抗结核药物治疗1-1.5年。结果:37例患者随访12-36个月,平均18个月。患者均腰背部疼痛消失,植骨融合,后凸畸形得到矫正。结论:侧前路双钉棒系统内固定治疗胸腰段脊柱结核既能彻底清除病灶,充分减压,又能矫正畸形、有效重建脊柱稳定性,防止神经损害,在治疗胸腰椎脊柱结核中具有重要的临床应用价值。Objective： To evaluate the effectiveness of operation of latero - anterior internal fixation by double nail - stick system of thoraeo - lumbar spinal tuberculosis. Methods： Thirty seven patients with spinal tuberculosis were treated by the operation of latero - anterior debridement, spinal cord decompression, bone graft and internal fixation by double nail - stick system, from Jan 2007 to Oct 2009. All the patients had different symptoms and accompanied with local pain and spinal kyphosis deformity with Cobb＇s angles of the thoraeolumbar spinal. 37 patients were constructed with bone graft and internal fixation by nail - stick system. Anti - tuberculosis chemotherapy regularly was given for 1 - 1.5 year after surgery. Results ： All the patients were followed - up for 12 - 36 months, average 18 months. All patients showed suecessful auto - graft bone graft fusion. The average eorreetion of kyphosis angle was 30°. Conclusion ： Operation of latero - anterior internal fixation by double nail - stick system of thoraco - lumbar spinal tuberculosis can cure tuberculosis and can also correct deformity, strengthen the spinal structure to prevent secondary paraplegia.
