摘 要:目的:探讨16层螺旋CT三维重建在胸部外伤骨折中的诊断价值。方法:利用16层螺旋CT容积扫描对临床怀疑骨折的60例患者进行后重建,运用容积再现技术(VR)及曲面重建技术(CPR)和多平面薄层重建技术(MPR)显示骨折的部位、数量,判断16层螺旋CT诊断胸廓骨折的价值。结果:60例患者中,共发现肋骨骨折259处,锁骨骨折13例,肩胛骨骨折16例,胸椎骨折8例,胸骨骨折2例。运用容积再现技术及曲面重建和多平面薄层重建技术可显示骨折的位置、数量及断端移位情况。各种后处理图像中,VR在显示骨折的空间解剖结构上有优势,CPR在显示微细肋骨骨折上效果较好,MPR像能从冠状位和矢状位判断解剖关系,而轴位图像是诊断的基础。结论:16层螺旋CT利用容积再现及曲面重建图像和多平面薄层重建可准确诊断不同程度的胸壁骨折,对治疗方案的选择及预后评估具有重要价值。Objective:To investigate the diagnosis of 16-slice spiral CT three dimensional reconstruction of the chest in traumatic fractures.Methods:Volume using 16-slice CT scanning for clinical suspected fracture of the 60 patients with reconstruction,using volume rendering(VR),and surface reconstruction(CPR) and multi-layer planar reconstruction(MPR) showed the site of fracture,the number Determined diagnostic 16-slice CT of thoracic fracture.Results:60 cases,rib fractures were found in 259,clavicular fracture in 13 cases,scapular fractures in 16 cases,thoracic fracture in 8 cases,sternum fracture in 2 cases.Using volume rendering techniques and surface reconstruction and multiplanar reconstruction showed the fracture of thin position,quantity and ends shifting.Various post-treatment image,VR showed fractures in the anatomical structure of space had an advantage,CPR fine rib fractures on the display was better,Coronal MPR image from the judge and the loss of anatomical shape spaces between,The axial image was the basis of diagnosis.Conclusion:Using 16-slice CT volume rendering and curved multiplanar reconstruction images,and thin-layer reconstruction can accurately diagnose different degrees of chest wall fractures,on the choice of treatment and prognosis of great value.
