小儿室间隔缺损及肺动脉高压的外科治疗

摘　　要：目的：报告了儿童室间隔缺损合并有其他畸形的手术方法及注意事项。方法：常规体外循环下进行手术，处理合并畸形时注意先后顺序。术后重视肺高压的处理。结果：３４３例手术中有３例死亡，均为肺高压。结论：肺高压和手术时机是与手术死亡率密切相关的两个因素。Objective:This article reviews the experience of surgical treatment of ventricular septal defect in children.Methods:Patients with ventricular septal defect underwent surgical repair under extracorporeal circulation from January 1990 to September 1998.There were 185 males and 149 females with a mean age of 11 9 years.54 patients had pulmonary hypertension before operation.Others associated with heart abnormalities including atrial septal defect in 13,patent ductus arteriosus in 14,and anomalous tricuspid valve and aortic valve in 20.Conclusions:Surgical treatment of ventricular septal defect in children should be performed as early as possible.Perioperative management is an important factor influencing surgical outcome.Pulmonary hypertension must be dealt with great care.
