犬晶体超声乳化和人工晶状体植入手术两种切口对比

摘  要:通过对比犬晶状体超声乳化同步植入人工晶状体2种切口在术中、术后的情况来探讨2种不同切口的差异性和护理要领,为本试验在临床上的应用找到一种合理的手术通路。本试验以犬为试验动物,观察透明角膜切口和巩膜隧道切口在超声乳化并同步植入晶状体的乳化时间,房水中TNF-α、H2O2和NO的含量;前房积血和角膜透明情况。结果显示,角膜透明切口的乳化时间(54.0s)显著低于巩膜隧道切口(61.5s)(P<0.05);同一时间透明角膜切口组与巩膜隧道切口组比较TNF-α、H2O2、NO含量差异不显著(P>0.05);巩膜隧道切口前房有不同程度的积血,术后15d有1只术眼仍未得到改善,而透明角膜切口组未出现前房积血。术后3d2种手术切口均出现角膜浑浊现象,15d后逐渐得以恢复。结果表明,2种手术通路术后均有明显的炎症反应和角膜浑浊;但角膜透明切口未出现前房积血且乳化时间更短,故手术并发症较少,适合犬白内障手术。In order to evaluate the differences between clear corneal incision and scleral tunnel incision of canine phacoemulsification and intraocular lens implantation,this test was designed to detect the time of phacemulsification,the concentration of TNF-α,H2O2 and NO in aqueous humor,and the postoperative status of cornea cloudiness and anterior chamber hyphema of the two incisions.As a result,the phacemulsification time of clear corneal incision was significantly shorter than that of scleral tunnel incision;the concentration of TNF-α,H2O2 and NO in aqueous humor had no significant difference between two groups;the scleral tunnel incision group had anterior chamber hyphema in different extent and one eye did not recover from hyphema untill 15th day after operation,but in the clear corneal incision group did not occur;the two incisions developed cornea cloudiness on 3rd day after operation,recovered after 15 days.The results showed that the both incisions had obvious inflammatory reaction and cornea cloudiness.However,the clear corneal incision had shorter phacemulsification time and no anterior chamber hyphema,indicating less operation complications.
