摘 要:目的探讨医院内白假丝酵母感染现状及对常用抗真菌药物的敏感性,为临床真菌感染的治疗提供一定的理论依据。方法收集临床真菌感染患者标本,进行菌株的分离、鉴定。分析白假丝酵母标本的来源和科室分布情况,并采用CLSI颁布的微量稀释法进行药物敏感性实验。结果白假丝酵母所占比例高达72.2%,主要来源于呼吸内科(65.6%),以痰标本的真菌分离率最高(86.8%)。药物敏感性实验结果表明,白假丝酵母对两性霉素B(AmB)、氟康唑(FCZ)、5-氟胞嘧啶(5-FC)、伊曲康唑(ICZ)的敏感率依次为100.0%、92.7%、91.4%、64.2%;仅对ICZ表现为耐药,耐药率为1.4%,但药物依赖性敏感高达34.4%。结论临床医生应高度重视医院内真菌感染;根据药物敏感性实验结果,合理用药,既能提高疗效,又可预防耐药性的产生。Objective To analyze the current conditions of nosocomial infection on Candida albicans and the susceptibility to common antifungal drugs,in order to provide a theoretical basis for the treatment of fungal infection.Methods The specimens of clinical infection of fungi were collected and the isolates were identified.Then the cases of Candida albicans were analyzed according to the source of samples and division distribution.Microdilution susceptibility testing was promulgated by CLSI.Results The infection rate of Candida albicans was as high as 72.2%.The infection rate of respiratory system was the most high(65.6%).The highest positive rate of sputum specimens was 86.8%.The results of susceptibility testing presented the sensitive rates of Candida albicans to AmB, FCZ,5-FC,ICZ were followed by 100%,92.7%,91.4%,64.2%;ICZ showed only resistance and the resistant rate to ICZ was 1.4%,but the sensitive rate of drug dependence was up to 34.4%.Conclusions The clinicians should think highly of the nosocomial fungal infections and use antifungal drugs on the rules of rational application.This will help to improve the curative effect and prevent drug resistance.
