腔内隔绝术治疗Stanford B型主动脉夹层的近期疗效分析

摘　　要：目的:探讨腔内隔绝术(endovascular graft exclusion,EVGE)治疗Stanford B型主动脉夹层(AD)的近期疗效。方法:对32例Stanford B型AD患者行EVGE。术后对所有患者定期随访并行CT检查,评价其临床改善程度。结果:32例患者均在全麻下成功进行了覆膜支架主动脉EVGE,术中造影显示动脉瘤破口均完全封闭,术后30d内死亡2例。术后随访1～72(18±16.9)个月,失访1例,术后出现左下肢体偏瘫1例,支架远端再次出现内膜撕裂口1例,Ⅰ型内漏3例,余24例无明显术后并发症。结论:EVGE治疗Stanford B型AD术中及术后并发症较传统开胸手术明显减少,术后近期疗效较好,远期疗效仍需进一步观察。Objective：To investigate the operation techniques and therapeutic effects of endovascular graft ex-clusion（EVGE）in the Stanford B type aortic dissection aneurysm.Method：Thirty-two patients of thoracic aortic dissection were treated by EVGE.The clinical and computer tomography outcome were analysed during follow-up. Result：All cases have technically successful EVGE.Complete disappearance of the false lumen or remarkable decrease of the endoleak was noted on the angiograms after stent placement.Two patients died in perioperation and one patient had loss of follow up.One patient suffered from left limb palsy.One patient had new tears.3patients had type I endlock after operation.And the other 24 patients didn＇t have severe complications duiring a 2-37 month follow-up.Conclusion：Compared with traditional operations,EVGE treatment for patients with Stanford B aortic dissection significantly decreased complications during and after operations.
