Harris-Galante Ⅰ非骨水泥型髋臼假体的中远期疗效分析

摘　　要：[目的]观察分析H/GI人工全髋关节置换术后髋臼假体中、远期疗效。[方法]对1992～2000年采用H/GI人工全髋关节置换术后髋臼假体置换21例(21髋)患者的临床病历和X线片进行回顾性研究。临床随访根据Harris进行评分,X线随访根据DeLee等髋臼分区进行观察。[结果]截止2010年5月10日,随访到19例(19髋),平均随访时间15年(10～18年)。19例中3髋臼杯进行了翻修,集中在术后9～12年。X线片示髋臼假体明确松动3髋、可能松动5髋,内衬磨损率为每年(0.27±0.10)mm。翻修手术中取出的假体表面的骨生长不满意。[结论]H/GI髋臼假体的中、远期疗效不满意。导致较高假体失效率原因包括内衬与金属臼杯设计不合理和过多的螺钉、螺钉孔有关的骨溶解与假体松动。[Objective]To observe the long-term clinical and radiographic outcomes of Harris-Galante Ⅰ（H/G I）acetabular prosthesis in total hip arthroplasty and to analyze the causes of its long period results.[Method]We retrospectively studied the patients records and X-ray films of 21 patients（21 hips） with H/G I acetabular prosthesis hip treated in the First Affiliated Hospital of Soochow University between 1992 and 2000.The clinical results were scored with Harris methods.The roentgenographic results were analyzed with Delee method.[Result]An average of 15-year follow-up（range 10 to 18 years） were available in patients（19 hips）.Among them,3 acetabular cups（1 hip in 1 male and 2 hips in 2 female） were revised.The average age of the patients was 64.3 years old（range 59-72 years）.Three hips were performed with acetabular cup revisions at 9,10 and 12 years post-operatively.There was positive loosening in 3 hips,probable loosening in 5 hips.All prosthesis liners wore in varying degrees.The liner wear rate was（0.27±0.10）mm per year.No good outgrowth bone was found on the surface of the cups taken off from the patients.[Conclusion]H/G acetabular prosthesis in total hip arthroplasty have poor mid-term and long-term results.The main causes for acetabular cup revision rate are poor design of acetabular cup and polyethylene liner,and osteolysis related to too many screws and screw holes.
