摘　　要：目的分析2型糖尿病患者肾小球滤过率(GFR)变化的相关因素。方法 120例2型糖尿病患者应用99mTC-DTPA法测量GFR,按GFR不同程度分为3组:A组GFR≥60 ml/min;B组30 ml/min≤GFR<60ml/min;C组GFR<30 ml/min。按血清肌酐(sCr)不同程度分为2组:sCr正常组,sCr≤120μmol/L;sCr升高组,sCr>120μmol/L。应用线性相关和多元逐步回归进行统计分析。结果①C组体质量指数(BMI)、sCr高于A、B两组(P<0.05),B、C两组SBP高于A组(P<0.05),三组间UAE差异无统计学意义。②在肌酐升高组,收缩压(SBP)、BMI、低密度脂蛋白(LDL)、总胆固醇(CH)高于正常组,GFR低于正常组(均P<0.05)。③多元回归分析结果显示:所有患者中血清肌酐,空腹血糖是2型糖尿病患者GFR下降的独立相关因素。结论 2型糖尿病患者GFR变化与诸多临床生化指标有关,其中与血肌酐和空腹血糖独立相关。Objective To analyze the related factors of glomerular filtration rate（GFR） in type 2 diabetes mellitus.Methods 120 diabetic patients were included.According to GFR evaluated by ^99mTC-DTPA levels,the patients were divided into three groups,group A with GFR more than 60ml/min;group B with GFR more than 30 ml/min,but less than 60 ml/min;group C with GFR less than 30 ml/min.Meanwhile,according to serum creatinine levels,the patients were divided into normal serum creatinine group（sCr≤120μmol/L） and abnormal serum creatinine group（sCr120μmol/L）.Results In group C,BMI and sCr were higher than those in group A and group B（P〈0.05）.In group A and group B,SBP was higher than that in group C.There was no diffirence on UAE among three groups.In abnormal sCr group,SBP,BMI,LDL and CH were higher than those in normal sCr group（P〈0.05）.While GFR was lower than that in normal sCr group（P〈0.05）.Stepwise multivariable regression analysis showed that the sCr（t=-3.649,P=0.001） and FPG（t=3.041,P=0.006） were independent correlation factors of GFR.Conclusion The change of GFR in type 2 diabetic patients is correlated with various clinical and biochemical parameters,sCr and FBG its are indepengent factors.
