微泵输注琥珀胆碱和瑞芬太尼静脉麻醉用于腹腔镜手术的研究

摘　　要：目的:观察瑞芬太尼、琥珀胆碱麻醉诱导,微泵持续输注实施静脉麻醉用于腹腔镜手术术毕的麻醉恢复情况。方法:选择腹腔镜下胆囊切除术、阑尾切除术及妇科异位妊娠切除术、子宫肌瘤剥除术等手术患者60例,依照麻醉用药及输注速率不同随机分为三组,每组20例。Ⅰ、Ⅱ两组为琥珀胆碱、瑞芬太尼静脉麻醉组;Ⅲ组(对照组),咪唑安定、芬太尼、维库溴铵、异氟醚静吸复合全麻组。结果:三组患者的年龄、身高、体重、ASA分级和手术时间无显著性差异(P>0.05)。自麻醉诱导始至手术结束停麻醉药后,患者自主呼吸恢复、苏醒、拔管、定向力恢复的时间Ⅰ、Ⅱ两组明显优于Ⅲ组,差异有统计学意义(P<0.05)。佳苏仑使用例数Ⅰ、Ⅱ两组亦明显少于Ⅲ组,差异有统计学意义(P<0.05)。结论:微泵输注琥珀胆碱和瑞芬太尼静脉麻醉用于腹腔镜手术术毕的麻醉恢复时间明显优于其他麻醉方式。Objective： To observe recovery from anesthesia after micropump infusion of Succinylcholine and Remifentanil for intravenous anesthesia in laparoscopic surgery.Methods： Sixty patients that received operation such as laparoscopic cholecystectomy,laparoscopic appendectomy,laparoscopic treatment of ectopic pregnancy and laparoscopic myomectomy were randomly divided into three groups according to anesthetic drugs and infusion rate,each group twenty patients.Patients in group Ⅰ and Ⅱ were used Succinylcholine and Remifentanil,while Ⅲ group（control group） used Dormicum,Fenthanyl,Vecuronium and Isoflurane inhalation anesthesia group.Results： The age,height,weight,American society of anesthesiologists and time for operation of patients in all three groups were close（P0.05）.From the beginning of anesthesia induction to stopping anesthesia,recovery of autonomous respiration,analepsia,extubation and recovery time of orientation of patients in group Ⅰ and Ⅱ were much better than those in group Ⅲ（P0.05）.The amount of patients who used Doxapram in group Ⅰ and Ⅱwere less than that in group Ⅲ and there existed obvious difference（P0.05）.Conclusion： The time of recovery from anesthesia after Micropump infusion of Succinylcholine and Remifentanil for intravenous anesthesia in laparoscopic surgery is shorter than others.
