尖锐湿疣患者皮损人乳头瘤病毒的基因分型及预后观察

摘　　要：目的了解东莞地区尖锐湿疣(CA)患者人乳头瘤病毒(HPV)感染型别,进一步探讨HPV基因型别和预后复发的相关性。方法采用核酸分子快速导流杂交基因芯片技术(Hybrin Max),对门诊确诊的200例CA患者的局部疣体标本进行HPV的21种基因型别进行检测,分型后对同一型别分为一组,采用同一或类似治疗方案,随访6个月并观察治疗后顽固性复发患者HPV型别情况。结果 200例CA患者中HPV阳性者153例,阳性率76.5%。在153例阳性患者中检出15种型别,其主要的基因型别及所占比例分别如下:HPV11(32.68%),HPV6(25.49%),HPV16(16.34%),HPV52(9.15%),HPV33(7.84%),HPV18(7.19%)。其中单一型HPV感染率为72.55%,混合型HPV感染率为27.45%。随访6个月复发3次及3次以上的患者共有21例,HPV主要为HPV16+18,HPV16+52,HPV16,HPV18+33,HPV6+11。结论东莞地区CA患者皮损HPV的基因型别依次为HPV11、6、16、52、33、18、58、42、45、31、35、41、44、51、68,以HPV11、6、16为主。混合型感染和高危型感染复发次数高,应引起我们临床重视。Objective To identify multiple human papillomavirus types in condyloma acuminatum（CA） lesions in Dongguan area,and to investigate the relation between virus types and prognosis.Methods 200 patients with CA were selected for human papillomavirus（HPV） subtype analysis using HybriMax.The patients with same HPV genotypes were given the same treatment,observe the genotypes of relapsing patients after six mouths.Results The total HPV positive rate was 76.5%（153/200）.15 different subtypes were found,and they were HPV11（32.68%）,HPV6（25.49%）,HPV16（16.34%）,HPV52（9.15%）,HPV33（7.84%）,HPV18（7.19%）.While the singe genotype infection rates were 72.55%,the multiple-HPV-genotype infection rates were 27.45%.21patients were recured three times or more after six months,the main HPV genotypes were HPV16＋18,HPV16＋52,HPV16,HPV18＋33 and HPV6＋11.Conclusion The main HPV genotypes of patients with CA in Dongguan area were HPV11,6,16,52,33,18,58,42,45,31,35,41,44,51,68,but HPV6,11,16 were the main genotypes.We should to pay more attention to the patients with multiple HPV genotypes for its high recurrence rate.
