摘  要:目的了解唐山地区弗氏志贺菌的耐药特征,明确其携带超广谱β-内酰胺酶(ESBLs)的基因型。方法采用纸片扩散法检测43株弗氏志贺菌的药物敏感性和ESBLs确证试验;对产ESBLs的菌株用微量肉汤稀释法测定其MICs;用PCR扩增ESBLs基因并测序。结果 43株弗氏志贺菌中,纸片扩散法检出具有ESLBs表型的菌株19株(44.2%),经琼脂稀释法检测,同样具有ESBLs表型;菌株对头孢曲松、头孢噻肟、头孢哌酮、哌拉西林、阿莫西林/克拉维酸和环丙沙星的不敏感率为21.95%~100.0%,对其他药物敏感率均>84.2%;经PCR扩增及测序,其型别主要为CTX-M-14、CTX-M-3、CTX-M-57和TEM-1。结论唐山地区儿童弗氏志贺菌呈多药耐药,对第三代头孢菌素耐药性有增加趋势,所携带基因型主要为CTX-M-14,首次检出CTX-M-57基因型。OBJECTIVE To analyze the drug resistance pattern and the extended-spectrum β-lactamase genotypes of clinical isolates of Shigella flexneri in Tangshan.METHODS Susceptibility testing was perfomed by K-B disk diffusion method and ESBLs confirmation experiment.The MICs against ESBLs-producting were determined by trace broth dilution method.PCR method was employed to detect the genotypes of ESBLs producing strains.The PCR products were analyzed by DNA sequencing.RESULTS A total of 19 ESBLs-producing strains were detected among 43 strains of Shigella flexneri,accounting for 44.2%.So did MICs.21.95%~100.0% of these isolates were susceptible to ceftriaxone,cefotaxime,cefoperazone,piperacillin and amoxicillin-clavulanate.More than 84.2% of these isolates were susceptible to other antibiotics.The genotypes of these stains included CTX-M-14,CTX-M-3,CTX-M-57 and TEM-1.CONCLUSION The Shigella flexneri isolates in Tangshan are multi-drug resistant,and the drug resistance to on the third generation cephalosporin antibiotic is increasing.The main genotype of ESBLs is CTX-M-14,and it is the first time to detect CTX-M-57 genotype.
