螺旋CT血管造影在脑动脉瘤诊断中的应用比较

摘　　要：目的:探讨螺旋CT血管造影(MSCTA)在脑动脉瘤诊断中的应用价值。方法:2008年1月～2009年12月我院收治怀疑为脑动脉瘤的患者30例,全部患者行螺旋CT血管造影检查,用最大密度投影(MIP)及容积重建(VR)法进行图像后处理,同期行数字减影血管造影(DSA)检查,将检查结果与DSA检查结果及手术结果对比,比较肿瘤检出的准确率。结果:30例患者中,术后证实脑动脉瘤33个,MSCTA检出动脉瘤31个,其中漏诊2个,与手术结果相符31个,诊断准确率为93.9%;DSA检出动脉瘤30个,其中漏诊2个,误诊1个,与手术结果相符30个,准确率为90.9%,两者比较,差异无统计学意义(P>0.05)。结论:MSCTA可以清楚地显示动脉瘤及其周围的解剖结构,诊断准确率高,无创、快速,可作为脑动脉瘤检查的首选方法。Objective： To evaluate the value of spiral CT angiography（MSCTA） in the diagnosis of cerebral aneurysms.Methods： From January 2008 to December 2009,30 patients suspected cerebral aneurysms were admitted to our hospital,all patients underwent spiral CT angiography,with maximum intensity projection（MIP） and volume rendering（VR） method for image post-processing,underwent digital subtraction angiography（DSA） inspection,the inspection results were compared with the results of DSA and operation,and the accuracy was compared between them.Results： 33 cerebral aneurysm were confirmed in 30 patients.31 aneurysms were detected by MSCTA,2 cases missed,31 cases were consistent with the surgical results,the diagnostic accuracy was 93.9%;DSA detected 30 aneurysms,of which 2 missed,1 misdiagnosed,30 were consistent with surgical findings,the diagnostic accuracy was 90.9%,the difference was not significant（P0.05）.Conclusion： MSCTA can clearly show aneurysm and the surrounding anatomy,the diagnostic accuracy is high,non-invasive,rapid,and can be used as the preferred method of cerebral aneurysms check.
