25例慢性心力衰竭患者炎症细胞因子的变化

摘　　要：目的观察慢性心力衰竭(CHF)患者外周血单个核细胞(PBMCs)核因子-κB(NF-κB)活化与炎症细胞因子表达的关系,探讨CHF的发病机制。方法选取心功能Ⅲ、Ⅳ级CHF患者25例,健康者25例,分离PBMCs,加入脂多糖(LPS),经体外培养24 h后,采用放射免疫法检测培养上清液中肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)和白细胞介素-1β(IL-1β)水平,并测定外周血单个核细胞NF-κB阳性细胞率。结果 CHF组IL-1β、IL-6和TNF-α的水平均比健康对照组明显升高(P<0.05),CHF组NF-κB阳性细胞率显著高于健康对照组(P<0.05)。相关分析显示,PBMCs NF-κB阳性细胞率与培养上清液中细胞因子TNF-α、IL-1β、IL-6水平呈显著正相关(r=0.54,P<0.000 1;r=0.53,P<0.000 1;r=0.55,P<0.000 1)。结论通过NF-κB活化途径而刺激炎症细胞因子水平升高,诱导心肌炎症反应,这可能是心力衰竭发生和发展机制之一。Objective To investigate the relationship between the secretion of cytokines and the activation of NF-κB in peripheral mononuclear cells（PBMCs） from patients with chronic heart failure（CHF）.Explore the possible mechanism of CHF.Methods PBMCs isolated from 25 CHF patients and 25 healthy subjects were stimulated by LPS in vitro for 24 hs.The supernatant fluid were used to measure the level of TNF-α,IL-1β,IL-6 by radioimmunoassay,and the activation of NF-κB in PBMCs were measured by immunohistochemical method.Results The secretion of cytokines（TNF-α,IL-1β,IL-6） and the activation of NF-κB in PBMCs showed a significant difference between CHF and healthy（P〈0.05）.The correlative analyses indicated that a positive correlation existed between the activation degree of NF-κB and the level of cytokines in PBMCs.Conclusion In patients with CHF,the secretion of cytokines may be regulated by NF-κB way.This property may be one of the mechanism for heart failure.
