慢性肾功能不全与B型钠尿肽的相关性分析

摘　　要：目的分析B型钠尿肽(BNP)在慢性肾功能不全(CRF)患者中的临床价值。方法选取2009年1月至2010年6月以慢性肾功能不全为主诊的肾内科住院的320例慢性肾功能不全(CRF)患者,将他们按肾小球滤过功能分为三组,并设立正常对照组100例。采用免疫荧光法测定患者全血BNP水平,分析BNP水平与患者肾功能状态的相关性。结果各组全血BNP水平分别为:正常对照组:(29.2±10.6)pg/ml;慢性肾功能不全(代偿期)(ICD码N18.003):(105.3±22.5)pg/ml;慢性肾功能不全(失代偿期)(ICD码N18.001):(373.6±86.7)pg/ml;慢性肾功能不全(尿毒症期)(ICD码N18.002):(1053.8±286.2)pg/ml。全血BNP水平随着肾小球滤过功能的下降而显著升高,差异具有统计学意义。结论慢性肾功能不全(CRF)患者全血BNP水平随着肾功能衰竭严重程度的增加而升高,BNP可作为一种简便,有效、快速的标志物用于慢性肾功能不全的危险分层。利用具有快速、简便,准确的BNP水平检测,对于提示慢性肾功能衰竭患者并发心血管疾病具有潜在的临床应用价值。Objective To study the clinical value of BNP in the patients who have been suffered from the chronic renal failure.Methods 320 patients with the chronic renal failure,who have been in hospital from January,2009 to June,2010,were divided into three groups according to the filtration function of the glomerulus and build a normal group.And then use the immunofluorescent method to test the BNP level,to analyze the relationship between the level of BNP and the renal function.Results The blood BNP level of different groups were as following：the level of the normal group is 29.2±10.6;the level of the patients with the chronic renal failure（compensation stage）is 105.3±22.5;the level of the patients with the chronic renal failure（discompensation stage） is 373.6±86.7;the level of the patients with the chronic renal failure（uremia stage）is 1053.8±286.2.The blood BNP level obviously raised up as the filtration function of the glomerulus went down,the difference shows the statistics meaning.Conclusion BNP can be a simple,quick,efficient marker to divide the different dangerous degrees of the chronic renal failure,and can give the hint to those patients with the chronic renal failure that it will be complicated by the cardiovascular diseases.
