利妥昔单抗注射液成功治愈1例难治性红斑型天疱疮

摘　　要：报告1例红斑型天疱疮患者,曾以糖皮质激素及免疫抑制剂治疗,皮损控制不佳,在给予利妥昔单抗注射液(美罗华)一个疗程(注射4次)治疗后,皮损消退且血清中抗体转阴,原治疗药物停用,至今病情无复发,达到了临床治愈的标准。血液中的B细胞在治疗后15个月完全恢复正常,治疗中和治疗后未出现不良反应。美罗华对于难治性自身免疫性大疱病有明确的疗效,不良反应相对较轻。A case of pemphigus erythematousus successfully treated with rituximab therapy is reported. The male patient was unresponsive to systemic corticosteroids and immunosuppressive drugs previously. So, one cycle of four weekly infusions of rituximab was given at a dose of 375 mg/m2 of body-surface area on days 1, 8, 15, and 22. The skin lesions were subsided and the serum autoantibody was negative after the treatment. During a 15-month follow-up period, the patient was free of disease with normal circulating B cells. Side effects were not observed in this case. It suggests that rituximab is an effective treatment for refractory case of pemphigus.
