摘  要:为评价超声背向散射参数判定心肌组织纤维化的临床价值。采用 H P S O N O S2500 型超声检查仪观测24 例陈旧性心肌梗死( O M I)患者梗死灶、38 例扩张型心肌病( D C M )和 45 例Ⅱ型糖尿病( D M)患者、正常对照组 40 例室间隔、左室后壁心肌的背向散射参数——背向散射积分( I B S)和 I B S的心动周期变化幅度( C V I B)。结果: O M I梗死灶、 D C M 和Ⅱ型 D M 患者心肌的校正 I B S( I B% )平均值均显著增大, C V I B均降低或出现 I B S曲线形态明显异常。 I B% 、 C V I B对 O M I、 D C M、 D M 均有较高的阳性预告值和阴性预告值。结论:背向散射参数对判定心肌病变、判断心肌纤维化程度较为客观、简便,有较高的临床应用价值。A clinical study in assessm ent of m yocardial fibrosis w ith ultrasonicbackscatter param eters Zhu Yongsheng, Qian Yunqiu, Zhou Xiaodong, Zhang Jun, Li Jun, Chen Yuxiang Departm ent of Ultrasound, Xijing Hospital, Fourth M illitary University, Xi′an 710032 Abstract   W e sought to assess the value of ultrasonic backscatter param eters in thedeterm ination of m yocardial fibrosis. M yocardial ultrasonic backscatter param eters of patientsw ith old m yocardial infarction ( O M I, 24 cases), dilated cardiom yopathy ( D C M , 38 cases),diabetes m ellitus ( D M ,45 cases) and 40 m em bers of norm al control group w ere m easured by acom m ercially available m odified realtim e backscatter signal 2 D im aging system ( H P S O N O S2500). Results: Tim eaveraged values of corrected m yocardial I B S ( I B% ) of previously infarctedm yocardium foci and I V S or L V P W of patients w ith D C M or D M w ere all significantly greaterthan those of the norm al. For m ost patients, patterns of tim e I B S curve turned out to beabnorm al and C V I B values decreased greatly. I B% , C V I B brought about high positive ornegative predictive values to O M I, D C M and D M . Conclusion: Ultrasonic backscatter w as usefulin determ ining m yocardial abnorm ality and assessing the degree of m yocardial fibrosis.
