北京市某社区2009年社区卫生诊断实例研究

摘　　要：目的发现并确定社区主要健康问题及危险因素,分析并提出社区需要优先解决的卫生问题,为制订社区卫生服务规划提供依据。方法采用现有资料收集和专项调查的方式收集社区健康相关资料。结果北京市某社区人口63000人,户籍人口14925人,流动人口48075人,流动人口占社区总人口的76.31%。常住人口35680人,占社区总人口的56.63%。2008年该社区户籍人口的主要死因为脑血管病(死亡率140.70/10万)、恶性肿瘤(87.10/10万)和心脏病(67.00/10万)。社区居民传染病报告发病率为252.38/10万,发病率居前3位的传染病为细菌性痢疾(63.49/10万)、其他感染性腹泻病(49.21/10万)、手足口病(36.51/10万)。社区儿童卡介苗接种率为99.02%,乙肝、脊髓灰质炎、百白破疫苗全程接种率为90.82%～93.88%。社区每千人口拥有卫生技术人员数为1.76人,而社区每千常住人口拥有卫生技术人员数为3.11人。结论社区需要优先解决的卫生问题是传染病预防与控制、加强妇幼保健工作和慢性病的预防控制工作。Objective To Find out and identify the main health problems and risk factors in the community, and to analyze and put forth the health issue necessary to be resolved urgently so as to provide the evidence for formulating the planning on community health service. Methods The secondary health related data were collected and specific survey was conducted to get primary data. Results This community has a population of 63 000, including permanent residents of 14 925, and transient population of 48 075 which account for the total population of 76. 31%. The de jure population of 35 680, which account for 56.63 % of the total population. The main death causes in this community were cerebrovascular disease （ the mortality is 140.70 per hundred thousand）, malignant tumors （the mortality is 87. 10 per hundred thousand） and heart disease （the mortality is 67.00 per hundred thousand）. The reported morbidity of infectious disease in the community was 252. 38 per hundred thousand. The rate of vaccination of BCG of children was 99.02%, the rates of vaccination of hepatitis B vaccine, poliomyelitis vaccine, and DPT were between 90. 82% and 93.88%. The ratio of medical technical personnel in the community was 1.76 per thousand population, and 3.11 per thousand de jure population. Conclusion The priority health related aspects of the community are the prevention and control of infectious diseases, strengthen maternity and child care, as well asprevention andcontrol of chronic diseases.
