2003—2009年102家医院住院患者抗菌药使用的趋势研究

摘　　要：目的科学评估住院患者抗菌药物使用变化趋势。方法应用统计描述和单因素分析方法分析2003—2009年间某省102家医院24.02万住院患者抗菌药物使用情况。结果 2003—2009年间住院患者抗菌药物总使用率为70.07%,使用率呈逐年下降趋势。给药途径以注射为主(92.16%),使用抗菌药物患者中平均使用2.66种抗菌药物。2003—2009年人均使用种类呈下降趋势。一级医院的抗菌药物使用率最高(73.15%),而细菌培养率最低(3.84%)。抗菌药物费用占使用了抗菌药物患者住院医疗总费用的17.53%,占西药费用的41.17%,这两个比例均呈下降趋势。结论 2003—2009年抗菌药物使用率略有下降,抗菌药物给药途径、给药种类、联合用药、细菌培养情况都有不同程度好转,但仍存在过度或不合理使用。Objective To scientificaly assess the usage trends of antibacterial drugs. Methods Statistical description and single - factor analysis were used to analyze the usage conditions of antibacterial drugs for 240 200 inpatients in 102 hospitals of a province from 2003 to 2009. Results The total prescribing rate of antibacterial drugs fro the inpatients was 70. 07% from 2003 to 2009, the prescribing rates decreased year by year, Most of their usage route was injection （92. 19% ）, averagely 2.66 kinds of antibacterial drugs were used. From 2003 to 2009 the prescribing kinds declined for each inpatient on average. The prescribing rate of antibacterial drugs in elementary hospitals was the highest （73.15%）, while the rate of bacterial culture was the lowest （3.84%）. The expense of antibacterial drugs accounted for 17.53% of the total expense of inpatients, accounted for 41.17% of the expense of western medicine, both the proportions decreased. Conclusion From 2003 to 2009 the prescribing rate of antibacterial drugs decreased slightly, the situations of administrative route, drug type, drug combination, and bacterial culture have been improved to a certain degree. Nevertheless, excessive and irrational usages of antibacterial drugs still existed.
