摘 要:目的探讨火器伤致犬颅内血肿的超声特征及其临床意义。方法应用常规超声及超声造影技术观察经CT证实的犬颅脑枪弹伤模型颅内血肿超声特征,并与病理结果对比。结果 (1)颅脑枪弹伤致颅内血肿发生率为50%。(2)常规超声显示其为脑实质内稍强回声团,脑中线及周围脑组织受压移位。(3)超声造影显示,血肿大部分区域早期呈完全无增强,其内或其周可见中断、穿行或绕行的高增强血管,随时间延长部分无增强区呈浅淡稍高增强,余血肿区域呈周边环状或者一侧壁条带状稍高增强,退出与正常脑实质一致。常规超声显示的部分血肿周边正常脑组织,造影模式下显示为充盈缺损区,即亦为血肿区。血肿以远脑实质呈等增强。结论常规超声易于判断血肿的位置、数量、大小及与周围脑组织关系。超声造影更能准确显示血肿实际范围,细致区分无活性区与活性区。其术中联合应用必将提高颅内血肿患者的救治水平。Objective To explore the value of ultrasound in the diagnosis and therapy of intracranial hematoma Induced by gunshot wound. Methods After establishment of the Canine model of penetrated cranioeerebral gunshot wound,we observed their ultrasound characteristics of the intracranial hematoma confirmed by CT using conventional ultrasound and CEUS (contrast enhanced ultrasonography) and then compared them with corresponding pathological examination. Results ①The incidence of intracranial hematoma Induced by gunshot wound is about 50% ②A slightly hyper echoic lesion could be seen in cerebral parenchyma by conventional ultrasound and it made surrounding brain tis- sues compressed. @The most part of hematoma areas showed non enhancement with the rest part of the lesion slightly hyper-enhanced by the CEUS in early and hypo enhancing vessels could entered into or bypass it. Some of nomenhance ment areas appeared slightly hyper-enhanced with the time prolonged. The normal brain tissues showed iso-enhance- ment. Conclusions Conventional ultrasound can easily judge the position, quantity, size of intracranial hematoma and the relationship between the surrounding cerebral tissue and it. CEUS can more precisely show the actual range of it and can distinguish the active area from inactive area carefully that is consistent with pathological examination. The application of ultrasound technology during operation will greatly improve the therapeutic and treatmental level of the patients.
