Evaluation of Left Ventricular Systolic and Diastolic Function in Patients with Congestive Heart Failure by Vector Flow Mapping

摘  要:目的探讨血流向量成像(VFM)技术在评价慢性心功能不全(CHF)左室收缩与舒张功能中的应用价值。方法选取CHF患者33例,正常对照44例为观察对象,利用VFM软件采集左室内血流向量图像,应用VFM技术测量收缩期心尖部~主动脉瓣口速度阶差(△Vs)、心尖部~主动脉瓣口距离(Ds)、半距对应速度(V_(S_(1/2)));舒张期二尖瓣口~心尖部速度阶差(△Vd)、二尖瓣口~心尖部距离(Dd)、半距对应速度(Vd_(1/2))。结果 (1)收缩期CHF组心尖部~主动脉瓣口速度阶差(△Vs)、半距对应速度(V_(S_(1/2)))与对照组相比,明显降低;(2)收缩期CHF组心尖部~主动脉瓣口距离(Ds)与对照组相比明显延长;(3)舒张期CHF组二尖瓣口~心尖部的△Vd降低、半距对应速度(Vd_(1/2))与对照组相比,明显降低;(4)舒张期CHF组二尖瓣口~心尖部(Dd)与对照组相比明显延长。结论 VFM技术可以无创地从血流动力学角度评价CHF患者的心脏功能状态,CHF患者的血流速度阶差明显低于正常人。Objective To evaluate systolic and diastolic function of left ventricle in the way of intracavitary flow dynamics by vector flow mapping with echocardiography. Methods 33 patients with congestive heart failure(CHF) and 44 healthy controls were involved. The velocity distribution imagines on the section plane of the flow in the left ventricle were obtained by vector flow mapping (VFM) ,and series parameters of velocity and distance were measured. Including systolic parameters, such as velocity gradient from cardiac apex to the aortic valve (AVs), distance from car dlac apex to the aortic valve (Ds), velocity according to half distance (Vs1/2), and diastolic parameters, such as velocity gradient from the mitral orifice to the apex (AVd), distance from mitral orifice to cardiac apex(Dd) ,velocity according to half distance(Vdl/2 ). Results (1) Compared to the control group, parameters of systolic velocity (△Vs, Vs1/2 ) in CHF group were lower. (2)Compared to the control group, parameters of systolic distance (Ds) in CHF group were longer. (3) Compared to the control group, parameters of diastolic velocity(△Vd, Vd1/2 ) in CHF group were lower. (4) Compared to the control group, parameters of diastolic distance ((Dd) in CHF group were longer. Conclusions Vector flow mapping could noninvasively demonstrate the flow structure of the ventricle and evaluate the cardiac function in patients with congestive heart failure.
