摘 要:目的评价冠心病心力衰竭(心衰)患者颈总动脉瞬时波强(wave intensity,WI)与左室收缩功能的关系。方法冠心病心衰患者24例,正常对照30例,采用Aloka Prosoundα10彩色多普勒超声诊断仪测定入选者右侧颈总动脉WI、左室射血分数(LVEF)、射血时间(ET)。结果冠心病心衰组W_1、NA、W_1-W_2及ET均小于对照组(均P<0.05),R-W_1大于对照组(P<0.001)。心衰组W_1与收缩压及LVEF均呈正相关(r=0.67、r=0.47,均P<0.05),W_1-W_2与ET及LVEF均呈正相关(r=0.63、r=0.77,均P<0.05),R-W_1与LVEF呈负相关(r= 0.63,P<0.05)。结论冠心病心衰患者WI减小,且与LVEF正相关,可作为临床评价左室收缩功能的一项指标。Objective To investigate the relationship of wave intensity(WI) and left ventricular systolic function in patients with heart failure because of coronary heart disease. Methods WI of right carotid arterial,left ventrieular e jection fraction(LVEF),ejection time(ET) were measured in 24 patients with heart failure because of coronary heart disease and 30 volunteers. Results W1,NA,W1-W2 ,and ET of heart failure group were significantly lower than those in the control group. R-W1 of heart failure group was significantly higher than that in the control group. In heart failure group,W1 was correlated significantly with systolic blood pressure and LVEF(respectively r=0.67, r=0.47 all P〈0.05). W1-W2 of heart failure group had significant positive relationship with ET and LVEF(respectively r 0.63,r=0. 77 all P〈0.05). R-W1 of heart failure group had significant negative relationship with LVEF(r=- 0.63, P〈0.05). Conclusions Wave intensity significantly decreases,which can be a clinic index to assess left ventricular systolic function.
