从络病论治系统性硬皮病417例临床观察

摘　　要：目的:探讨以络病理论为指导辨分型治疗系统性硬皮病的临床疗效。方法:将近3年来住院的417例患者分5个证型采用中药辨证治疗,分别为寒邪阻络、肺卫不宣型,寒凝腠理、脾肾阳虚型,痰浊血瘀阻络型,气血两虚、脉络失荣型和湿热阻络型,并结合对症输液及口服西药治疗,观察其总有效率及患者甲襞微循环和关节功能治疗前后的变化。结果:总有效率94.24%,甲襞微循环(形态积分、流态积分、袢周积分)和关节功能(疼痛指数、肿胀指数、功能指数)治疗前后均有显著性差异(P<0.01)。结论:络病论治系统性硬皮病疗效确切,从络病论治系统性硬皮病应做为一个基本治则贯穿治疗始终。Objective：To discuss clinical curative effect of treatment for systemic scleroderma typed under guidance of the theory of collateral disease. Methods：treated recent 3-year 417 in-patients based on differentiation of syndromes,divided them into five types：type of cold obstructing collaterals and failure of releasing stagnated lung-energy,type of cold stasis in striae of skin,insufficiency of both the spleen and the kidney,type of stagnation in collaterals ,stagnation in collaterals due to ropy phlegm and blood stasis,type of deficienty of both qi and blood ,failure in nourishing collaterals stagnation in collaterals due to cold and heat. Expectant treatment of drip and oral western medicine was combined. Observe total effective rate and the change of microcirculation of nail fold and articular function before and after treatment. Results：the total effective rate is 94. 24% . There are significant differences in both microcirculation of nail fold （morphologic integral,hydrokinetic integral,integral） and articular function （pain index,swell index,function index） before and after treatment（P 0. 01） . Conclusion：There is positive curative effect of treatment of the theory of collateral disease for systemic scleroderma. The diagnosis and treatment of systemic scleroderma based on collaterals should de used as principle throughout the treatment.
