摘  要:目的:观察揉髌手法对兔膝关节软骨细胞凋亡的影响,探讨手法治疗的作用机制。方法:随机将50只新西兰兔分为手法组、药物组、模型组、假模组和正常组,每组10只;前3组手术造成骨内高压型模型,假模组手术但不损伤相应血管,正常组不做任何处理。手法组每只兔揉髌手法治疗17次,药物组膝关节内注射玻璃酸钠注射液5次,其他兔无特殊处理。造模后12周末处理所有动物,取右膝内侧胫骨平台软骨,采用免疫组化法检测软骨细胞凋亡基因Bcl-2、Bax、Fas表达,统计学处理。结果:手法组Bcl-2表达(2.67±0.50),模型组(0.33±0.52)(两组比较P<0.01);手法组Bax(1.30±0.67),模型组(2.44±0.73)(两组比较P<0.01);手法组Fas(1.56±0.73),模型组(2.67±0.52)(两组比较P<0.01)。结论:揉髌手法通过上调Bcl-2,下调Bax、Fas基因表达减少兔膝关节软骨细胞凋亡,可能是延缓兔膝关节软骨组织退变的机制之一。Objective:To observe the Rou-Bin Manipulation on apoptosis of knee joint chondrocytes in rabbit,explore the influ-ence of manipulation mechanisms. Methods:50 New Zealand rabbits devieded randomly into five groups:Manipulation group,drug group,Model group,dummy-model group and the normal group,10 rabbits in each group; the first 3 groups cause IOH-model by operative,dummy-model group operate without damage the corresponding vessel,no treatment for the normal group. 17 times of Rou-Bin manipulation on each rabbit in manipulation group,the rabbit injection with sodium hyaluronate 5 times in drug group,no special treatment for other rabbits. 12 weeks after modeled,to take all the rabbit right knee medial tibial plateau cartilage tissue processing,detected chondrocyte apoptotic gene Bcl-2、Bax、Fas expression by using immunohistochemical method,statistics processing. Results:Bcl-2 expression result(2. 67 ± 0. 50) for manipulation group,(0. 33 ± 0. 52) for model group(P 0. 01 to compare with both groups) ; Bax expression result(1. 30 ± 0. 67) for manipulation group,(2. 44 ± 0. 73) for model group(P 0. 01 to compare with both groups) ; Fas expression result(1. 56 ± 0. 73) for manipulation group,(2. 67 ± 0. 52) for model group(P 0. 01 to compare with both groups) . Conclusion:The Rou-Bin manipulation reduced apoptosis of knee joint chondrocytes in rabbit through up-regulating Bcl-2 expression and down-regulating Bax、Fas expression,possibly one of the mechanism of delay the degeneration of cartilage tissue of knee joint in rabbit.
