摘 要:目的分析小儿呼吸机相关性肺炎(VAP)的主要病原菌,从医学的角度探讨预防呼吸机相关性肺炎发生的措施。方法回顾分析2000年1月~2003年12月机械通气≥48h,发生VAP71例患儿气管插管下取痰作细菌培养和药敏试验。结果 71例VAP患儿取标本148份检出病原菌86株,革兰氏阴性菌占76.7%。其中肺炎克雷伯菌、铜绿假单胞菌、不动杆菌为主要病原菌,近年金葡菌也成为感染主要病原菌。结论革兰阴性菌是小儿机械通气的主要病原菌,近年金葡菌感染有增多趋势,而VAP病原菌耐药性多,发病机制复杂,提高预防意识,采取综合防治措施,是控制呼吸机相关性肺炎的最佳策略。Objective Analysis of the child in the relevant pneumonia(VAP) main pathogen from medicine to examine the prevention of respiratory the relevance of pneumonia.Methods The review analyzed in January,2000-December,2003 machinery ventilation ≥48h,had under the VAP71 example baby trachea canella to take the phlegm to make the bacilli culture and the medicine sensitive experiment.Results 71 example VAP baby takes specimen 148 to pick out disease germ 86,the Gram-negative bacteria accounts for 76.7%,and pneumonia gram Rebar fungus,the verdigris pseudomonad,the motionless bacillus are the main disease germs,near annuity Portugal fungus also becomes the infection main disease germ.Conclusion The leather blue negative fungus is the young child machinery ventilation main disease germ;near annuity Portugal fungus infection has increases the tendency,but the VAP disease germ drug resistance are many,the pathogenesis is complex,raises the prevention consciousness,takes the integrated control measure,controls the life-support machine relevant pneumonia's optimum strategy.
