二至调经颗粒对多囊卵巢综合征不孕患者卵泡质量的影响

摘　　要：目的探讨二至调经颗粒对多囊卵巢综合征(PCO S)不孕患者卵泡质量的影响。方法将60例PCO S不孕患者随机分为两组,每组30例。两组患者均以炔雌醇环丙孕酮预处理3个周期,第4个周期开始对照组予克罗米酚+注射用尿促性素,治疗组在此基础上口服二至调经颗粒。观察两组患者肾虚证候的改善情况;比较两组治疗后妊娠率、卵巢基质血流、雌二醇(E2)、黄体生成素(LH)的差异以及血清中血小板活化因子(PAF)含量的差异,观察其与卵泡质量的相关性。结果治疗组妊娠率36.7%,对照组26.7%,两组妊娠率比较无统计学差异(P>0.05)。治疗组肾虚证候的改善明显优于对照组(P<0.05);治疗组在改善卵巢基质血流方面,提高E2、PAF水平均显著优于对照组(P<0.05)。对LH改善两组无统计学差异(P>0.05)。结论二至调经颗粒合西药在提高肾虚型PCO S不孕患者的卵泡质量及妊娠率方面有良好的疗效,其机理与改善内分泌水平及卵巢基质血流情况有关,同时与影响血清PAF的产生有一定内在联系。Objective To investigate the effect of Erzhi Tiaojing Granules（Herbal granules for regulating menstruation） on the follicle quality in infertility patients with polycystic ovary syndrome（PCOS）.Methods Sixty PCOS patients were randomized into two groups,30 in each.Both groups were pretreated with ethinylestradiol and cyproterone acetate tablets for three circle.At the begining of the fourth circle,both groups were given clomifene citrate capsule and menotrophin for injection,while the treatment group was additionally given Erzhi Tiaojing Granules.After treatment,the improvement of kidney deficiency symptoms in the two groups was observed,the difference in pregnancy rate,blood flow of stroma of ovary,estradiol（E2）,luteinizing hormone（LH）,and the content of serum platelet activation factor（PAF） were compared to see their correlation with the follicle quality.Results The pregnancy rate of the treatment group and the control group was 36.7% and 26.7% respectively with no statistic difference between（P0.05）.The improvement of the treatment group in kidney deficiency symptoms was significantly better than that of the control group（P0.05）.The treatment group was superior in the improvement in blood flow of stroma of ovary and increase of E2 and content of PAF（P0.05）.There was no statistical difference between groups in the improvement in LH（P0.05）.Conclusion Erzhi Tiaojing Granules in combination with western drug is effective in improving the follicle quality and pregnancy rate of PCOS patients with kidney difficeincy in TCM.The mechanism of which is related to improving the endocrine system and blood flow of stroma of ovary,at the same time,to its effect on regulating the PAF production.
