活血利湿法与健脾利湿法治疗慢性下肢静脉性水肿临床对比研究  

Comparative Study on Blood-activating Damp-dissolving Method with Spleen-strengthening Damp-dissolving Method in the Treatment for Chronic Venous Edema of Lower Limbs

摘　　要：目的比较活血利湿法与健脾利湿法治疗慢性下肢静脉性水肿的临床疗效。方法将50例慢性下肢静脉性水肿患者随机分为两组,活血组30例服用祛湿消肿胶囊,健脾组20例服用参苓白术丸,疗程均为8周,观察治疗前后中医症状、体征评分及彩色超声测定下肢固定点皮肤及皮下组织厚度的变化情况。结果治疗后活血组愈显率66.67%,总有效率86.67%;健脾组分别为35.06%、60.00%,活血组的愈显率和总有效率明显高于健脾组(P<0.05);两组症状总分、体征总分均有下降,相比较活血组下降更为明显(P<0.05);两组患者皮肤及皮下组织厚度均有减少(P<0.05或P<0.01),但组间比较均无统计学差异(P>0.05)。结论活血利湿法治疗慢性下肢静脉性水肿疗效优于健脾利湿法。Objective To compare the effect of blood-activating damp-dissolving method and spleen-strengthening damp-dissolving method on chronic venous edema of lower limbs.Methods Fifty cases with chronic venous edema of lower limbs were divided into two groups.The blood-activating group,30 cases,was administered Qushi Xiaozhong Capsules（Capsule for dissolving damp and relieving edema）,while the spleen-strengthening group,20 cases,was given Shenling Baizhu Wan（Bolus of Ginseng,Poria and Bighead Atractylodes）,for 8 weeks.The changes in the scores of TCM symptoms and signs,and the thickness of skin and subcutaneous tissue in the specific areas of lower limbs detected by the Color Doppler Ultrasound examination were observed.Results After treatment,curative and remarkably effective rates（66.67%） and total effective rate（86.67%） of the blood-activating group were significantly higher than those of the spleen-strengthening group,being 35.06% and 60.00% respectively（P0.05）.The scores of symptoms and signs of both groups were lowered,but those of the blood-activating group were more significant（P0.05）;the thickness of skin and subcutaneous tissue in both groups was reduced,but there was no significant difference between groups（P0.05）.Conclusion The blood-activating damp-dissolving method is better than the spleen-strengthening damp-dissolving method in effect on chronic venous edema of lower limbs.
