摘 要:目的研究产超广谱β-内酰胺酶(ESBLs)大肠埃希菌和肺炎克雷伯菌中Ⅰ类整合子的分布,探讨整合子介导的耐药机制在产ESBLs菌耐药性中的作用。方法用PCR检测100株产ESBLs大肠埃希菌和肺炎克雷伯菌Ⅰ类整合酶基因及blaTEM、blaSHV和blaCTX-M基因,对Ⅰ类整合子阳性菌进行耐药基因盒检测并测序。结果产ESBLs菌株中Ⅰ类整合子阳性率为69%,整合子阳性的产ESBLs菌株对包括头孢菌素类、喹诺酮类、磺胺类和β-内酰胺酶抑制剂类在内的12种抗菌药物的耐药率高于整合子阴性产ESBLs菌株,2χ检验差异有统计学意义,P<0.05;Ⅰ类整合子在blaSHV、blaTEM和blaCTX-M基因阳性菌株中检出率分别为84.4%、72.9%和68.9%。结论在产ESBLs大肠埃希菌和肺炎克雷伯菌中,Ⅰ类整合子广泛分布且参与产ESBLs菌的多重耐药的形成,容易引起多重耐药的广泛传播。Objective To study the distribution of class I integron in extended spectrum β-lactamases(ESBLs)-producing Escherichia coli(E.coli) and Klebsiella pneumoniae(K.pneumoniae) and investigate the mechanism of integron-mediated multi-drug resistance of ESBLs-producing strains.Methods The presence of class I integron and blaTEM,blaSHV and blaCTX-M genes for coding class I integron were detected by PCR among 100 ESBLs-producing E.coli and K.pneumoniae.The gene cassettes in class I integron were detected and sequenced.Results The percentage of expression of class I integron was found as 69% among ESBLs-producing strains.Comparing to those strains without class I integron expression,the strains with class I integron presented significantly higher resistance to 12 antibiotics(P0.05),including cephalosporins,quinolones,sulfonamides and β-lactam enzyme inhibitors,etc.The expressions of class I integron were 84.4%,72.9% and 68.9% in the stains with blaSHV,blaTEM and blaCTX-M.Conclusions Prevalent distribution of class I integron in ESBLs-producing E.coli and K.pneumoniae was found.Integrons showed definite influence on the development of multi-drug resistance.
