摘 要:目的从痰液成分的定量研究来探讨三组不同气道湿化法的利弊。方法随机选取我院气管切开术后患者54例,作为痰液标本收集的对象。分别采用定时法、持续法及定时加雾化法后,测定痰液成分中α-酸性糖蛋白、Ca2+含量和pH值。结果定时加雾化组痰液成分中α-酸性糖蛋白、Ca2+含量低于定时组、持续组中的含量,三组比较差异有极显著意义(P<0.01)。三组气道湿化中pH值的分布图显示,在机体正常范围内,定时加雾化组高于其他两组,并呈现升中求稳的气道需求趋势。结论三组气道湿化方式在满足呼吸道正常生理需求即衡量湿化效果的指标之一的基础上,本研究中定时加雾化组相比较于定时组、持续组能够较有效的降低痰液成分中影响痰液黏稠度的α-酸性糖蛋白、Ca2+含量,实现气道湿化的最终目标即分泌物的吸引通畅和及时清除。Objective To analysis the advantages of three different tracheal humidification techniques from the quantitative investigation of sputum ingredients. Method Fifty-four patients with tracheotomy was chosen as the experimental objects and divided randomly and equally into three groups: regular group(RG) using the regular tracheal humidification technique, Experimental group(EG) using the continuous tracheal humidification technique and Im- provement group(IG) using regular humidification method together with oxygen atomization technique. The three humidification methods are further analyzed by observing the variations of sputum ingredients including a-acidic glycolprotein, Ca^2+ and pH value. Result Compared with RG and EG, IG obtains lower e-acidic ghycoprotein and Ca^2+ content with the most significant statistical effect among three groups (P〈0.01). Besides the pH value achieving from IG is higher than those from RG and EG. Conclusion By satisfying the respiratory tract's normal physiology, which is one of targets for trachea humidification, regular humidity together with oxygen atomization technique is superior to the continuous tracheal humidification technique and regular humidification technique in respect of reduc- ing a-acidic glycoprotein and Ca^2+ content of sputum ingredient, which further reduces the sputum viscidity so as to implement secretion suction in a unobstructed and clear way.
