戊酸雌二醇配伍卡前列甲酯栓在绝经后妇女取宫内节育器中的应用

摘　　要：[目的]探讨应用戊酸雌二醇配伍卡前列甲酯栓在绝经后妇女取宫内节育器(IUD)中的效果,从而为提高绝经术后取环成功率提供进一步指导。[方法]200例自愿要求取环而无禁忌症的绝经后妇女进行手术,根据用药方法随机分为观察组和对照组各100例,其中观察组给予,对照组给予,观察比较两组手术时间及宫颈软化程度、取环的成功率及手术中的疼痛反应程度。[结果]观察组的宫颈软化的效果明显优于对照组,经统计学处理,差异有统计学意义(P﹤0.05)。观察组顺利取出的所占比率明显高于对照组,其中困难取出的比率,包括分别采取扩宫、扩宫+搔刮、剪断+抽拉等手术方法的发生率和失败率均明显低于对照组,经统计学处理,差异有统计学意义(P﹤0.05)。观察组的不同程度的疼痛反应与对照组分别比较,差异均有统计学意义(P﹤0.05)。[结论]戊酸雌二醇配伍卡前列甲酯栓在绝经后妇女取宫内节育器中合用,可以明显减轻取IUD的患者疼痛程度,提高取IUD的成功率,降低取IUD的失败率,取环效果显著,方法简便易行,适合基层医疗单位应用。[Objective]To investigate the effect of forefront of estradiol valerate combined with methyl card tied in postmenopausal women who was taking intrauterine device,so as to improve the success rate of IUD after menopause to provide further guidance.[Methods]200 postmenopausal women voluntary requested to take part without contraindications to conduct surgery.According to administration methods,these cases were randomly divided into two groups and the control group,each group had 100 cases.The operation time,cervical softening degree,and the success rate and extent of surgery in the pain response were compared between the observation group and the control group.[Results]The cervical softening of the observation group was significantly better than the control group（P﹤0.05）.The successful rate of taking intrauterine device in the observation group was significantly higher than that of the control group.The incidence of surgical failure in the observation group was significantly lower than that in control group（P﹤0.05）,including dilatating uterus,dilatating uterus ＋ scraping,cut ＋ pull,etc.The degrees of pain responses in observer Group was significantly lower than that in control group（P﹤0.05）.[Conclusion]The estradiol valerate combined with cards tied the forefront of methyl intrauterine device in postmenopausal women who was taking intrauterine device can significantly reduce pain in patients with IUD,improve the successful rate of taking IUD,and this method is simple and suitable for application of primary health care units.
