摘　　要：目的:探讨应用Bard3DMax免钉合补片行完全腹膜外腹腔镜腹股沟疝修补术(totally extraperitoneal prosthesis,TEP)的安全性及有效性,总结其临床应用经验。方法:回顾分析2008年9月至2009年9月应用Bard 3DMax补片免钉合行腹腔镜TEP治疗38例腹股沟疝患者的临床资料。38例中斜疝21例,直疝7例,复合性疝6例,双侧疝3例,复发疝1例。结果:38例手术均获成功,无一例中转开放手术,手术时间40～150min,术后住院3～7d,平均4d。3例发生血清肿,2例抽液后治愈,1例自行吸收,所有病例术后均无疼痛及阴囊血肿发生。术后随访8～20个月,无复发病例。结论:应用Bard 3DMax补片免钉合TEP安全有效,具有术后患者康复快,并发症少,复发率低等优点,值得临床推广和应用。Objective：To study the safety and efficacy of totally extraperitoneal laparoscopic inguinal prosthesis （TEP） with Bard 3DMax free-stapling patch,and summarize clinical experience.Methods：A retrospective analysis was made on the clinical data of 38 patients undergoing TEP for inguinal hernia with Bard 3DMax free-stapling patch from Sep.2008 to Sep.2009.It included 21 cases of indirect hernia,7 cases of direct hernia,6 cases of compound hernia,3 cases of bilateral hernia and 1 case of recurrent hernia.Results：All operations were successful,without conversion to open operation.Operative time was 40-150min,and postoperative hospital stay was 3-7d,with an average of 4d.There were no patients suffering from postoperative pain or scrotal hematoma.3 cases got seroma,2 of which were healed after drainage and 1 was self-limited.Within the follow-up of 8-20 months,no recurrence occurred.Conclusions：TEP with Bard 3DMax free-stapling patch is a safe and effective surgery with advantages of more rapid recovery,fewer complications and lower recurrence rate.It is worthy of being generalized.
