摘　　要：目的：探索ＥＢＣＴ 仿真支气管镜的成像方法和成像效果，并初步探讨其应用价值。材料和方法：５０ 例正常人群和３０ 例伴气管、支气管病变者。所有研究对象均接受ＥＢＣＴ 薄层扫描，其横断面图像转入ＩＮＳＩＧＨＴ 工作站进行三维重建。其后选用ＳＩＭＵＬＡＴＥＥＮＤＯＳＣＯＰＹ 程序进行仿真支气管镜。结果：运用ＥＢＣＴ 仿真支气管镜：５０例正常人群清晰地显示了气管、支气管内表面的正常形态与表现， 显示支气管腔可达段一级水平；１８ 例中央型肺癌患者显示腔内肿块的形态及内表面或支气管腔狭窄、阻塞，１２ 例气管、支气管良性狭窄患者分别显示病变的范围、形态及内腔表面的异常表现。３０ 例病例均可以显示病变部位以远的支气管管腔情况。结论：ＥＢＣＴ 仿真支气管镜可以清楚地显示气管、支气管内腔的正常或异常表现；可用于部分气管、支气管良、恶性病变的诊断。ＥＢＣＴ 仿真支气管镜提高了ＥＢＣＴ 用于气管、支气管病变的诊断能力，是对纤维支气管镜的补充。Purpose:To introduce the method and imaging effect of virtual bronchoscopy with EBCT(Electronic Beam Computed Tomography)and preliminarily explore its application value.Materials and Methods:50 cases of normal group and 30 cases with tracheo-bronchial abnormality group were analyzed.All the objects got EBCT examination with thin-section scan.The cross-section images were transmitted to the INSIGHT workstation and reconstructed with SSD (Shadow Surface Display),and then the 3D images were transfered into virtual bronchoscopy with SIMULATE ENDOSCOPY package.Results:EBCT virtual bronchoscopy revealed that the normal interior surface of trachea and bronchi(to segmental level)could be demonstrated clearly in 50 cases of normal group, 18 cases of central airway cancer were also shown,including the tumor's shape and their surface or obstruction of the airway,12 cases with benign stenosis of tracheobronchial tree showed the stenotics longitudinal range,shape and abnormal appearance of their interior surfaces.With EBCT virtual bronchoscopy,the bronchial cage distal to the lesion were revealed easily in all the latter 30 cases.Conclusion:EBCT virtual bronscopy can reveal clearly the normal or abnormal inner surface of central airway,it is helpful to differentiate some benign and malignant lesions of the tracheobronchial tree. EBCT virtual bronchoscopy may sometimes be complement to fibrobronchoscopy.
