改良B-lynch缝合术在剖宫产术中发生难治性产后出血中的应用

摘　　要：目的探讨改良B-lynch缝合术在难治性产后出血中的临床实用价值。方法回顾性分析2005年1月～2009年9月在剖宫产术中发生的难治性产后出血80例,按手术方式分为两组,45例采用改良B-lynch缝合术治疗,35例采用宫腔填塞纱布术治疗。比较两组的术中出血及术后出血量、止血效果、术后产褥病率、需要行二次手术治疗等情况。结果改良B-lynch缝合术治疗组和宫腔填塞纱布术治疗组两组在术中及术后48h出血量、止血效果、术后产褥病率发生率、需行二次手术治疗例数方面差异有显著性(P<0.05)。结论改良B-lynch缝合术是治疗难治性产后出血的有效方法,易于操作,值得推广。Objective To assess the value of improved B-lynch suture in the treatment of the intractable postpartum hemorrhage during cesarean section and find a new method of the treatment of the intractable postpartum hemorrhage during cesarean section.Methods We retrospectively reviewed 80 women who encountered intractable postpartum hemorrhage during cesarean section,in the department of obstetrics of people＇s hosptial of Pingdu from Jan.2005 to September 2009,according to hemostasis method used,they were divided into the improved B-lynch suture group（n=45） and the uterine packaging group（n=35）,The patients＇ conditions during and after the operation were compared between the two groups.Results in the respect of average intraoperative and postoperative 48h blood loss,case of second operation,the difference between the two groups had statistical significance（P0.05）.Conclusion Improved B-lynch suture in the treatment of the intractable postpartum hemorrhage during cesarean section is effective,easy to operate and worthy to be popularized.
