摘 要:目的探讨组织多普勒速度模式评价高血压病右室舒张功能的应用价值。方法选择69例轻中度高血压患者及60例健康志愿者,应用脉冲组织多普勒测量e(三尖瓣环舒张早期峰值速度)、a(舒张晚期峰值速度),计算e/a;应用脉冲多普勒测量三尖瓣血流频谱E(舒张早期峰值速度)、A(舒张晚期峰值速度),计算E/A。比较正常组与高血压组间的差别,并对组织多普勒与脉冲多普勒有关参数进行相关分析。结果正常组e/a>1,E/A>1;高血压Ⅰ、Ⅱ组与正常组比较,E、E/A、e、e/a均降低,而Ⅰ组中a、A无变化,Ⅱ组中a、A均增高。Ⅱ组与Ⅰ组比较:E、A、e、a无明显变化,e/a及E/A明显减低。正常组e、a及e/a均与年龄相关(r=-0.23,P<0.05;r=0.72、-0.65,P<0.01)。高血压Ⅰ组中a、e/a及Ⅱ组中e/a与年龄密切相关(r=0.34,-0.58,-0.45;P<0.05)。E/A与e/a在正常组及高血压组均相关。结论高血压病早期即出现右室舒张功能减退,组织多普勒检测三尖瓣环运动速度可以反映右室舒张功能的改变,并优于脉冲多普勒。[Objective] To assess right ventricular diastolic function in essential hypertension by DTI (Doppler Tissue Imaging). [ Methods ] The peak e (early) and a (late) diastolic velocities of tricuspid annulus in 69 patients with essential hypertension and 60 controls were obtained. At the same time, E (the peak early) and A (late) diastolic flow velocities, and E/A of tricuspid valve were analyzed. [ Results ] The E, E/A, e and e/a in patients were lower (P 〈0.05) compared with the control group. The a and A had no change in Ⅰ group (P 〉0.05). The a and A were higher in Ⅱ group. The e, a and e/a were correlated to age in controls. The e/a correlated with E/A in controls and patients. [ Conclusion ] The right ventrieular diastolic function is altered in essential hypertension, DTI can quantify the changes of tricuspid annular velocity in hypertension. DTI is a simple and practical method in evaluation of right ventrieular diastolic function.
