脑卒中患者家庭照顾者抑郁及社会支持的纵向研究

摘　　要：目的调查脑卒中患者家庭照顾者抑郁和社会支持状况,分析动态变化及相互关联,为循证护理提供依据。方法采用抑郁量表(CES-D)和社会支持量表(MSPSS)对164例脑卒中患者的家庭照顾者于患者出院前,出院后3周、12周及24周进行调查分析。结果照顾者在这4个时间点的抑郁发生率分别为48.2%、44.5%、39.6%及42.1%,照顾者在患者出院后12周、24周的抑郁水平显著低于患者住院期间(P<0.05);社会支持得分分别为4.93分、5.18分、5.22分及5.29分,照顾者在出院后感知的社会支持显著高于出院前(P<0.01);CES-D得分在10分及以上的照顾者感知的社会支持显著低于得分在10分以下者(P<0.05)。结论脑卒中患者家庭照顾者感知的社会支持不充足,但抑郁较常见,他们感知的社会支持水平越低,越趋向于出现抑郁症状。Objectives To investigate depression and the levels of social support among family caregivers of stroke survivors,and to analyze the dynamic change and their correlation.Methods A total of 164 stroke survivor-caregivers were investigated with Center for Epidemiologic Studies Depression Scale（CES-D） and Multidimensional Scale of Perceived Social Support（MSPSS） during hospitalization and at 3 weeks,12 weeks and 24 weeks after discharge.Results The incidence of depression among family caregivers of stroke survivors was 48.2%,44.5%,39.6% and 42.1% during hospitalization and 3 weeks,12 weeks,24 weeks after discharge,respectively.The scores of depression at 12 weeks and 24 weeks after discharge were significant lower than that during hospitalization（P0.05）.The score of social support was 4.93,5.18,5.22 and 5.29 at the four time points,respectively.It was significant higher after discharge than during hospitalization（P0.01）.The score of social support among depressed caregivers was significanlty lower than that of those not depressed（P0.05）.Conclusions The social support perceived by the family caregivers of stroke survivors is insufficient.Depression is prevalent among family caregivers.The caregivers with lower level of social support are more likely to be depressed.
