摘  要:针对患者安全危害,提出在认识问题上注重系统问题、不良事件上报和建立安全文化;质量目标与行为的关系、质量从"问题"开始、质量常态和"柔软质量管理"的理念;持续质量改进,主要是过程改进、持续性改进和预防性改进等三个方面分别阐述了安全、质量与持续质量改进理念。结合医院实践,介绍了改进质量管理体系、推行安全主题培训、引入第三方促进、实施临床路径、开展全院性安全落实活动以及建立数据、分析与持续改进制度的做法与成效。Based on patient' s safety, conceptions such as focusing on systematic problem, defect event report and building security culture, relations between quality objective and human behavior, quality is from question, regular quality management and soft quality management were mentioned. Relationship between continuous quality promotion, safety and quality was discussed in views of process promotion, continuous promotion and preventive promotion. Combined with hospital experience, experiences and effectiveness on quality management system promotion, security training, third party inspection, clinical pathway, security measures implementation, creating database, analyzing and continuous promoting system were discussed.
