健康成年人血小板聚集功能调查

摘　　要：目的建立杭州地区健康成年人血小板的聚集功能的参考范围。方法以不同终浓度的二磷酸腺苷(ADP)(2μmol/L,1μmol/L)和肾上腺素(EP)(5.56μmol/L,2.78μmol/L)为诱导剂,采用光学比浊法分别检测241名健康志愿者的血小板的聚集率,求出1 min时及最大聚集率值。另选32例心肌梗死患者进行血小板聚集功能的检测。结果以终浓度2μmol/L,1μmol/L ADP为诱导剂时,正常人群血小板的1 min聚集率分别为(43.35±9.05)%,(37.04±5.32)%;最大聚集率分别为(60.83±4.29)%,(55.91±6.13)%。以终浓度5.56μmol/L,2.78μmol/L EP为诱导剂时,正常人群血小板的1 min聚集率分别为(26.74±9.82)%,(22.26±9.41)%;最大聚集率分别为(58.12±8.50)%,(45.65±9.78)%。以ADP为诱导剂时,心肌梗死患者组的1 min及最大聚集率均显著高于正常对照组;以EP为诱导剂时,心肌梗死患者组的最大聚集率明显高于正常对照组。结论本实验建立了杭州地区健康成年人群血小板聚集功能的参考范围,可为临床高凝状态的监测以及抗凝药物疗效的观察提供参考依据。Objective To establish the reference range of platelet aggregation function in healthy adults of Hangzhou city.Methods Platelet aggregation rate in 241 healthy volunteers was tested by optical nephelometery,using adenosine diphosphate（ADP）（2 μmol/L,1 μmol/L） and epinephrine（EP）（5.56 μmol/L,2.78 μmol/L） of different final concentrations as inducer.The 1 minute and maximal aggregation rate were measured.Additionally platelet aggregation of 32 patients with myocardial infarction was tested.Results When induced by ADP with final concentration of 2 μmol/L and 1 μmol/L,the 1 minute platelet aggregation rate of healthy adults was（43.35±9.05）% and（37.04±5.32）% respectively,and the maximal aggregation rate was（60.83±4.29）% and（55.91±6.13）% respectively;when induced by EP with final concentration of 5.56 μmol/L and 2.78 μmol/L,the 1 minute platelet aggregation rate of healthy population was（26.74±9.82）% and（22.26±9.41）% respectively,the maximal aggregation rate was（58.12±8.50）% and（45.65±9.78）% respectively.The 1 minute and maximal aggregation rate of patients with myocardial infarction induced by ANP was significantly higher than controls;While induced by EP,the maximal aggregation rate of patients with myocardial infarction was significantly higher than controls.Conclusion This study established the reference range of platelet aggregation function in healthy adults of Hangzhou city,which provided reference for monitoring hypercoagulation status and observing the efficacy of anticoagulants in clinical practice.
