垂体后叶素和催产素在腹腔镜下输卵管间质部妊娠术中的应用对比

摘　　要：目的:探讨垂体后叶素与催产素在腹腔镜下输卵管间质部妊娠术中止血效果。方法:选择67例血流动力学稳定的输卵管间质部妊娠为研究对象,行腹腔镜手术,其中35例(A组)行患侧输卵管切除和宫角切开取胚术前在病灶周围注射垂体后叶素6U加生理盐水10mL,32例(B组)行患侧输卵管切除和宫角切开取胚术前在病灶周围注射催产素20U加生理盐水10mL。结果:67例手术均在腹腔镜下完成。术中出血量:A组(42.6±7.7)mL,B组(64.2±18.0)mL;手术时间:A组(48.0±8.3)min,B组(64.8±21.6)min。A组缝合止血6例,B组缝合止血17例。A组出血量明显少于B组(P<0.05),手术时间较B组明显缩短(P<0.05)。结论:腹腔镜下输卵管间质部妊娠术中应用垂体后叶素止血比应用催产素止血效果要好。OBJECTIVE To explore the hemostasis of pituitrin and oxytocin in operations of interstitital pregnancy under laparoscope. METHODS Operation was performed under laparoscope for 67 cases of interstitital pregnancy, among which 35 cases were in group A （ the pituitrin group） and the other 32 cases were in group B （control group）. The patients in the group A were injected with 6U pituitrin and 10 mL NaCI of 0. 9% concentration in horn of uterus or lesions around before salpingectomy or tubal open admission of embryos, The patients in the group B were injected with 20 U oxytocin 10 mL NaCl of 0. 9%. RESULTS All the operations for the 67 cases were performed under laparoscope. The amount of bleeding during operation was （42.6± 7.7）mL in group A, （64. 2± 18. 0）mL in group B. The operation time was （48. 0± 8. 3）min in group A, （64. 8 ±21.6） min in group B. Suture hemostasis was performed in 6 cases of group A and 17 cases of group B. The amount of bleeding of group A was significantly less than that of group B （P〈0. 05）, the operation time of group A was significantly less than that of group B （P〈0. 05）. The rate of suture hemostasis in group A was significantly lower than that in group B. CONCLUSION The application of pituitrin in operations of interstitital pregnancy under laparoscope can is a more effective hemostasis than that of oxytocin.
