中药组方对腹腔镜保守手术治疗输卵管妊娠结局的影响

摘　　要：目的:观察腹腔镜保守手术后联合应用中药组方对输卵管妊娠结局的影响。方法:将198例腹腔镜保守手术治疗的输卵管妊娠患者随机分为三组(A、B、C),A组(手术+术后中药组方治疗)55例,B组(手术+术中MTX15mg患侧输卵管系膜内注射治疗)57例,C组(单纯腹腔镜保守手术)86例。结果:A、B两组均无PEP发生,C组PEP发生率为10.47%。A、B两组发生率明显低于C组(P<0.05)。A、B两组术后96h血β-hCG下降率明显高于C组(P<0.05)。B组治疗后不良反应发生率明显高于A、C两组(P<0.05)。结论:腹腔镜保守手术后联合应用中药组方治疗输卵管妊娠,可明显加快术后血β-hCG下降速度;可预防PEP的发生,其有效率与MTX组相同,且不良反应发生率明显降低,值得临床推广应用。Objective： Observation on impact of Chinese traditional medicine formulae on postoperative tubal pregnancy treated by conservative laparoscopy.Method：198 cases of tubal pregnancy treated by conservative laparoscopy were randomly divided into 3 groups（A,B,C）,group A（patients accepted operation and Chinese traditional medicine formulae after operation） with 55 cases;group B（patients accepted operation and 15mg methotrexate was injected in mesenterium to the ipsilateral tubal during operation） with 57 cases;group C（only operation,regarded as control group） with 86 cases.Result：There was no occurrence of persistent ectopic pregnancy in group A and B,the incidence of persistent ectopic pregnancy in group C was 10.47%.The incidence of persistent ectopic pregnancy in group A and B was obviously lower than that of group C（P0.05）.The rate of decline of β-HCG in blood in 96 hours after operation in group A and B was obviously higher than that of group C（P0.05）.The incidence of adverse effect in group B was obviously higher than that of group A and C（P0.05）.Conclusion：Chinese traditional medicine formulae is used on postoperative tubal pregnancy treated by conservative laparoscopy,which can obviously accelerate the speed of decline of β-HCG in blood,which can prevent the occurrence of persistent ectopic pregnancy,whose efficacy is the same as the methotrexate group,but lower adverse effect,which is worth to be applied clinically.
