住院精神病人合并肺结核X线影像学表现及临床分析

摘　　要：目的:了解精神病人合并肺结核影像学特点临床表现及临床对策。方法:对我院2000年1～2008年12期间14610份出院病历进行筛查,共查出61人感染了肺结核,并进行回顾性调查分析。结果:61例中3例为Ⅱ型肺结核(4.92%),46例为Ⅲ型肺结核(75.4%),9例为Ⅳ型肺结核(14.75%),3例为Ⅴ型肺结核(4.92%)。住院精神病人合并肺结核发病率明显高于常人肺结核发病率,肺部X线表现较常人同类病人严重治疗后不同时期X线变化较常人同类病人恢复慢,且合并症多。结论:精神病人合并肺结核以Ⅲ型肺结核为最多,且病情严重,对精神病人合并肺结核应尽早检查、早诊断、早规范治疗,防止交叉感染及暴发流行。Objective： To understand clinical manifestations and clinical solution of hospitalized mental patients complicated with pulmonary tuberculosis, Methods： Screen 14610 discharged medical records from January, 2000 to December, 2008, and find out 61 patients who were infected with pulmonary tuberculosis, and do retrospective analysis. Results： In the 61 cases, 3 cases （4.92%） are type Ⅱ pulmonary tuberculosis, 46 cases （75.4%） are type Ⅲ pulmonary tuberculosis, 9 cases （14.75%） are type IV, and 3 cases （4.92%） are type Ⅴ. The pulmonary tuberculosis morbidity of hospitalized mental patients is higher than ordinary persons. And for the same type pulmonary tuberculosis, hospitalized mental patients are more serious than ordinary patients according to the chest X-ray imaging manifestations. After treatments, hospitalized mental patients recovered more slowly than ordinary patients according to X-ray imaging of different times and had many com- plications. Conclusions： For hospitalized mental patients complicated with pulmonary tuberculosis, the type III pulmonary tuberculosis is the most mad they are in a serious condition. And for them, it is better to examine, diagnose and treat normally as early as possible.
