妊娠合并糖尿病对母婴的影响研究

摘　　要：目的分析妊娠合并糖尿病孕妇血糖控制情况对母婴预后的影响。方法回顾性分析妊娠合并糖尿病患者150例资料,按照血糖控制情况分为两组,每组75例。观察两组妊娠结局与新生儿情况。结果研究组(血糖控制良好组)妊娠期高血压综合征、羊水过多、胎膜早破、感染发生率显著低于对照组(血糖控制不良组),而产后出血和高脂血症的发生率两组差异无显著性。研究组低血糖、巨大儿、宫内窘迫、头颅B超异常发生率均显著低于对照组,但新生儿黄疸、吸入性肺炎发生率两组差异无显著性。两组终止妊娠方式和分娩时孕周差异具有高度显著性(P<0.01)。结论血糖控制情况直接影响妊娠合并糖尿病患者与新生儿预后,孕期血糖控制可减少母婴并发症。一旦确诊妊娠合并糖尿病,应尽早在医师的指导下开始有规律地饮食控制或胰岛素治疗。Objective To analyze the effect of blood glucose control in pregnant women complicated situation on the maternal prognosis.Methods The data of 150 cases of diabetes during pregnancy were retrospectively analyzed,and divided into two groups according to blood sugar control,one group 75 cases.Outcome of pregnancy and newborns were observed.Pregnancy and neonatal outcome were observed situation.Results Study group （good blood sugar control group） pregnancy induced hypertension,polyhydramnios,premature rupture of membranes,infection rate was significantly lower than the control group （poor glycemic control group）,while postpartum hemorrhage and the incidence of hyperlipidemia was no significant difference between two groups.Study group with low blood sugar,great children,fetal distress,abnormal head B-were significantly lower,but neonatal jaundice,the incidence of aspiration pneumonia was no significant difference between two groups.Two ways to terminate pregnancy,and gestational age at delivery was significant difference（P〈0.01）.Conclusion Direct impact on blood sugar control in patients with gestational diabetes and neonatal outcome,glycemic control during pregnancy can reduce maternal and infant complications.Once the diagnosis of gestational diabetes should be under the guidance of a physician as soon as possible begin to diet or regular insulin.
