超敏C反应蛋白与β_2-微球蛋白检测在糖尿病肾病早期诊断中的意义

摘　　要：[目的]探讨血清超敏C-反应蛋白(Hs-CRP)和β2-微球蛋白(β2-MG)对糖尿病肾病早期诊断的应用价值。[方法]采用免疫比浊法测定47例正常蛋白尿、37例微量蛋白尿、31例临床蛋白尿的糖尿病患者血清Hs-CRP、β2-MG的含量,与40例正常对照组进行比较。[结果]糖尿病组血清Hs-CRP与β2-MG明显高于对照组(P<0.01)。血清Hs-CRP在正常蛋白尿组(3.05±0.79)mg/L明显高于正常对照组(1.46±0.57)mg/L(P<0.01);在微量蛋白尿组(6.72±1.79)mg/L与临床蛋白尿组(13.82±5.27)mg/L明显高于正常蛋白尿组(3.05±0.79)mg/L及正常对照组(1.46±0.57)mg/L(P<0.01)。血清β2-MG在微量蛋白尿组(3.35±1.05)mg/L与临床蛋白尿组(10.61±4.04)mg/L明显高于正常蛋白尿组(1.34±0.40)mg/L及正常对照组(1.17±0.25)mg/L(P<0.01)。糖尿病肾病患者血清Hs-CRP与β2-MG显著正相关(r=0.758,P<0.01)。[结论]血清Hs-CRP与β2-MG是诊断糖尿病早期肾脏微血管病变的敏感指标,同时检测二项指标对糖尿病肾病的早期诊断与病情监测有重要的临床意义。[Objective] To explore the application value of serum high-sensitivity C-reactive protein（Hs-CRP） and β2-microglobulin（β2-MG） in the early diagnosis of type 2 diabetic nephropathy.[Methods] Serum β2-MG and Hs-CRP of 47 cases with normal proteinuria,37 with microalbuminuria and 31 with proteinuria of diabetic patients were determined by immunoturbidimetric assay,and 40 healthy subjects served as controls.[Results] The levels of Hs-CRP and β2-MG in the diabetic patients were significantly higher than those in normal control group（P0.01）.The levels of Hs-CRP in patients with normal proteinuria（3.05±0.79） mg/L were significantly higher than those in control group（1.46±0.57）mg/L,（P0.01）.The levels of Hs-CRP in patients with microalbuminuria（6.72±1.79）mg/L and with proteinuria of diabetic（13.82±5.27）mg/L were significantly higher than those in patients with normal proteinuria（3.05±0.79）mg/L and control group（1.46±0.57）mg/L,（P0.01）.The levels of β2-MG in patients with microalbuminuria（3.35±1.05）mg/L and with proteinuria of diabetic（10.61±4.04）mg/L were significantly higher than those in patients with normal proteinuria（1.34±0.40）mg/L and control group（1.17±0.25）mg/L,（P0.01）.Serum β2-MG and Hs-CRP in diabetic nephropathy patients were significantly correlated（r=0.758,P0.01）.[Conclusion] Serum Hs-CRP and β2-MG are sensitive indicators in early diagnosis of renal microvascular lesion.Detection of them is important in the early diagnosis of diabetic nephropathy and disease supervision.
